Garfield Avenue Alhambra, CA 92418 REQUEST The Buddhist Tzu-Chi Free Clinic, a non-profit organization, is proposing to hold a health fair on July 14, 1996 at 1441 and 1450 N. "D" Street. The Clinic provides one-stop outpatient care, including doctor treatment, lab test X-ray service and medicine to needy persons. The Buddhist Tzu-Chi Free Clinic intends to submit an application for a temporary use permit on July 9, 1996, and pay all applicable fees for this event. Pursuant to Section 3.13.02 of the Municipal Code, the Clinic is requesting a refund of$298, the TUP application fee. RECOMMENDATION Staff recommends that the Mayor and Common Council authorize a refund of $298. D St. San Bernardino,CA 92418 Date:July 5, 1996 Dear Valerie: Per our conversation on July 3, 1996,we,Buddhist Tzu-Chi Free Clinic,would like to apply of waiving the Temporary Use Permit application fee of $298.00. We will submit the fee when we turn in the application form.However,we would like to ask your support to assist us to process the fee waivability at next council meeting. Buddhist Tzu-Chi Free Clinic is a California Nonprofit Public Benefit corporation. Our mission is trying to provide the medical relief support to the needy group who has no second or third resources of medical care. Buddhist Tzu-Chi Free Clinic will provide a one-stop outpatient care to the patient who qualified for this service that includes free doctor treatment,lab test,X-ray service and medicine. We are planning to hold our 2nd services to the residents of San Bernardino.The first medical service was held on February 5, 1996 at our Clinic(1000 South Garfield Ave., Alhambra,California 91801).There were more than 388 San Bernardino residents accept our services. This time we plan to move our medical team to your city by providing the direct services to them.One reason will be solved the transportation for your residents, and the other reason will be looking into the possibility of set up a satellite clinic in your neighborhood. We realize this application form has submitted too late and may cause some internal process problem to your city. Please try to bear with us. "D" STREET Dear Ms. Boudreaux: The above referenced Temporary Use Permit is approved for July 14, 1996, for the purpose of holding the requested health clinic. The proposed use has been found to be consistent with Section 19.36.050 of the Development Code, Findings. The approval is subject to compliance with the following Conditions of Approval: 1. The temporary use shall not extend beyond July 14, 1996. 2. Hours of operation are limited to 8 am until 4 pm. 3. The applicant shall be responsible for obtaining any other permits/approvals, including any outside agencies, which may be required for this temporary use. 4. Permits for the tents are required from the Fire Department. The applicant is subject to compliance with all Fire Code requirements. 5. The applicant will utilize the waste disposal bins on-site. 6. The restroom facilities at the Catholic Charities buildings will be available for the public for the duration of this approval. 7. Temporary off-street parking spaces shall be maintained for the duration of the event. 9. The temporary use shall be operated in a manner which does not produce excessive noise, vibration, odor, dust, smoke, glare, or any other nuisance. 10. The Clinic shall provide security for the duration of the event. 11. The Clinic shall use security/clinic volunteers to help with street crossings and any other specified uses. In particular, the volunteers shall be stationed in front of each site to ensure that people cross at the intersection and not mid-block. 12. You are required to post surety guaranteeing the availability of$500.00 (to the satisfaction of the City Engineer), to ensure that the site is returned to its original condition by July 15, 1996. This is a refundable deposit. I have prepared a request for a refund of the Temporary Use Permit fee of$298. It is scheduled for consideration by the Mayor and Common Council at their meeting of July 15, 1996. Assuming that your request is approved, a check will be prepared and mailed to you.
