Synopsis of Previous Council Action:
08-07-95 -- Authorization to Proceed was given to initiate proceedings for the formation of an acquisition
assessment district for the construction of New Pine Avenue and confirm the selection of
Brown, Diven and Hentschke as legal consultant, and GFB-Friedrich & Associates as
Assessment Engineer.
09-05-95 -- Resolution No. 95-328 was adopted authorizing execution of agreements with consultants.
12-18-95 -- Resolution 95-435 was adopted authorizing execution of Addendum No.l to an agreement
with GFB-Friedrich & Associates for Assessment Engineering services relative to the
formation of Assessment District No. 1015.

STAFF REPORT
On 8-7-95, authorization was given to initiate the formation of an Acquisition Assessment
District for the construction of New Pine Avenue, between Kendall Drive and Irvington Avenue.
The selection of Brown, Diven and Hentschke for Legal Consultant and GFB - Friedrich and
Associates for Assessment Engineer were also confirmed at that meeting and on September 5, 1995,
Resolution No. 95-328 was adopted, approving the execution of an agreements with the consultants.
The agreement with GFB - Friedrich provides for Assessment Engineering services,
including preparation of the Engineer's Report, the District Boundary and District Diagram Maps
and related tasks, for a fee not to exceed $13,900.
On December 18, 1995, Addendum No. 1 to this agreement was approved. This was
necessary to allow for the monitoring of progress payments and work progress, which was not part
of the original agreement. It was estimated that 5 such reviews would be needed. Due to inclement
weather during the construction period, construction time was extended, resulting in the need for
additional reviews of the work progress and payment requests. The increase in the contract was for
$1,552.
The attached Resolution approves the execution of Addendum No. 2 to the existing agreement,
to include the additional reviews of the progress payment billings. This will result in an increase
of $ 504 to the existing contract.

ADDENDUM NO. 2
TO EXISTING AGREEMENT
ASSESSMENT DISTRICT NO. 1015
(New Pine Avenue)
Addendum No. 2 to the existing Agreement, dated September 7, 1995, by and between the City
of San Bernardino (hereinafter called CITY) and GFB-Friedrich & Assoc. (hereinafter called
ENGINEER), for the services required to form Assessment District No. 1015 (New Pine
Avenue), is comprised of the additional work required to provide a total of nine (9) construction
site visits and nine (9) associated reviews of construction payment requests instead of three (3)
of each as anticipated in Addendum No. 1. The additional work and associated fee increase
shall be based on the terms and conditions set forth in said existing Agreement.
Within the general guidelines of assessment district law regarding acquisition projects, the
ENGINEER shall provide the additional services listed below:
Provide the services necessary to complete a total of nine (9) monthly site visits
and nine (9) monthly progress payment reviews since the project began. These
services are estimated to be sufficient to take the project to completion.
Sr Civil Engr 6 hours/month @ $84.00/hour *504.00
Therefore an additional amount of $504.00 shall be added to the existing contract amount for
forming and administering Assessment District No. 1015.
IN WITNESS WHEREOF, the parties have caused Amendment No. 2 to the referenced
Agreement to be executed on the day of 19 by their
respective officer duly authorized in that behalf.
