05-01-89 - - Resolution No. 89-119 adopted awarding a contract to
Inland Overhead Door Company, for the low bid price of
$27 , 721 . 00 .
06-19-89 - - Change Order No. One approved, to authorize bringing
the overhead doors up to good working condition, for
an increase of $9 , 453 . 75 in the contract price.
07-02-90 - - Change Order No. Two approved, to extend the contract
for a one-year period, from 7-1-90 to 6-30-91 , at no
adjustment in the contract prices .
04-24-91 - - Change Order No. 3 approved, to extend the contract
for a one-year period, from 7-1-91 to 6-30-92 , at no
adjustment to the contract prices .

04-06-92 - - Change Order No. 4 approved, to extend the contract
for a one-year period, from 7-1-92 to 6-30-93 , at a
reduction of $8 ,516 . 00 in the contract price.
03-22-93 - - Resolution No. 93-68 adopted authorizing execution
of an amendment to the contract to extend the term
for a period of 1 year, from 7-1-93 to 6-30-94 , at
no increase in the contract price; and to incor-
porate 12 screens of Carousel Mall into the
contract.
04-29-94 -- Resolution No. 94-126 adopted authorizing execution
of an amendment to the contract to extend the term
for a period of 1 year, from 7-1-94 to 6-30-•95 , at
no increase in the contract price.
04-15-95 -- Resolution No. 95-149 adopted authorizing execution
of Third Amendment to contract, at no increase in
the unit prices.

CITY-OF SAN BERNARDINO - REQUEST FOR COUNCIL ACTION
STAFF REPORT
Inland Overhead Door Company has a contract with the City
to maintain electrical and manually operated roll-up doors at
various City buildings . This is a highly specialized mainten-
ance, which the Contractor has performed in a very satisfactory
manner.
An inquiry was sent to this Contractor asking if they
would be willing to accept another extension of one year on this
contract and, if so, under what conditions. Inland Overhead Door
Company responded that they would be willing to accept an
extension on their contract, at no increase in the unit prices.
Costs incurred for services under this contract will be
charged to Object Code 5507 , "Building Maintenance, " for the City
facilities listed below:

1 FOURTH AMENDMENT TO CONTRACT
2 This Contract Amendment, made and entered into this
3 day of , 19 , by and between the City of San
4 Bernardino, a Municipal Corporation, 300 North "D" Street, San
5 Bernardino, California 92418 (City) and INLAND OVERHEAD DOOR
6 COMPANY, A California Corporation, 12401 South La Cadena Drive,
7 Colton, California, 92324 (Contractor) .

10 WHEREAS, the parties hereto entered into that certain
11 Contract dated May 1 , 1989 , relating to the provision of
12 Maintenance of Overhead Doors at Various Locations , per Plan No.
13 7663; and
14 WHEREAS, Change Order No. 2 extended the contract from
15 July 1 , 1990 , to June 30 , 19 91 , Change Order No. 3 extended the
16 contract from July 1 , 1991 , to June 30 , 1992 , and Change Order
17 No. 4 extended the contract from July 1 , 1992 , to June 30 , 1993 ;
18 and
19 WHEREAS, the amendment to contract as authorized by
20 Resolution No. 93-68 , adopted on 3-22-93 , extended the term from
21 7-01-93 to 6-30-94 , and added 12 screens at Carousel Mall to the
22 contract; and Resolution No. 94-126 , adopted on 4-29-94 , extended
23 the term from 7-1-94 to 6-30-95 , at no increase in the contract
24 price.
25 WHEREAS, Amendment Number Three to contract extended the
26 term from 7-01-95 to 6-30-96 , at no increase in the contract unit
27 prices; and

FOURTH AMENDMENT TO CONTRACT: REPAIR AND MAINTENANCE OF OVERHEAD
DOORS
j WHEREAS , the parties desire now to amend said Contract to
2 extend the term of the Contract.
3 NOW, THEREFORE, the parties agree, as follows :
4 A. The Contract shall be extended for a period of one
5 year from July 1 , 1996 , to June 30 , 1997 , with no increase in the
6 contract unit prices .
7 B. Article I of the Agreement is amended to require
8 that Contractor shall perform in accordance with Standard
9 Specifications for Public Works Construction, 1994 Edition.
10 C.- Section 6-1 . 01 , Sound Control Requirements , is
11 amended to require Contractor to comply with Chapter 8 . 54 of the
12 San Bernardino Municipal Code regulating and prohibiting loud,
13 unnecessary and excessive noise.
14 D. Except as amended herein, all other terms ,
15 specifications and conditions of the original agreement and first
16 amendment shall remain unchanged and in full force and effect.
