1 RESOLUTION NO.
2 RESOLUTION OF THE MAYOR AND COMMON COUNCIL OF THE CITY OF
SAN BERNARDINO APPROVING A FIRST AMENDMENT TO MEMORANDUM OF
3 UNDERSTANDING (AMBULANCE TRANSPORT SERVICES)
4 WHEREAS, on May 1, 1995, the Mayor and Common Council approved that certain
5 Memorandum of Understanding with Courtesy Services of San Bernardino, Inc., by Resolution
6 No. 95-136, and
7 WHEREAS, the parties thereto desire now to amend said Memorandum of
8 Understanding relating to the term thereof,
9 NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND COMMON
lo COUNCIL OF THE CITY OF SAN BERNARDINO AS FOLLOWS:
11 SECTION 1. The Mayor is hereby authorized and directed to execute that certain First
12 Amendment to Memorandum of Understanding (Ambulance 'Transport Services) between the
13 City of San Bernardino and Courtesy Services of San Bernardino, in a form as attached hereto
14 as Exhibit "A" .
15 SECTION 2. The authorization to execute the above-referenced Memorandum of
16 Understanding is rescinded if the parties to the Memorandum of Understanding fail to execute
17 it within sixty (60) days of the passage of this resolution.

1 FIRST AMENDMENT TO
MEMORANDUM OF UNDERSTANDING
2 (AMBULANCE TRANSPORT SERVICES)
3 This First Amendment to Memorandum of Understanding is made and entered into this
4 day of , 1996, by and between the City of San Bernardino (hereinafter
5 referred to as "City"), and Courtesy Services of San Bernardino, Inc. (hereinafter referred to as
6 "Courtesy");

8 WHEREAS, the parties hereto entered into that certain Memorandum of Understanding
9 dated May 4, 1995, which by its terms took effect on May 1, 1995, and
10 WHEREAS, the parties desire now to amend said Memorandum of Understanding relating
11 to its term,

13 NOW, THEREFORE, the parties hereto agree as follows:
14 I. Section "I" of the Memorandum of Understanding as noted above is hereby amended
15 to read as follows:

17 "I. Term:
18 The agreement set forth in this Memorandum of Understanding shall
19 take effect on May 1, 1995, and will continue until July 31, 1996, at which
20 time it may be continued on a month-to-month basis at the discretion of the
21 Mayor of the City as conveyed in writing to Courtesy prior to any
22 expiration thereof, and with the agreement of Courtesy.
23 City and Courtesy shall review the terms and provisions hereof on
24 a regular basis to insure that they remain in conformity with all applicable
25 laws."

27 II. Except as amended herein, said Memorandum of Understanding shall remain in full

3 force and effect.
4 IN WITNESS WHEREOF, the parties hereto have executed this First Amendment to
5 Memorandum of Understanding n the day and date first above shown.
