Description: SD-A Finance: Council Notes: Agenda Item No._' 2. 5".4.oq , CITY OF SAN BERNARDINO - REQUEST FOR COUNCIL ACTION STAFF REPORT Subject: Authorize the Director of Development Services to execute Contract Change Order No. One with Guzman Construction Co. for the reconstruction of the Storm Drain and Catch Basin at Highland A venue and "H" Street. Background: On October 13, 2008, a request for an informal bid was sent to Weka Inc., Nikola Construction Corporation, and Guzman Construction. The informal bid request was to repair a box culvert at Highland Avenue and "H" Street. Guzman Construction was the only contractor that responded to the bid at a price of $24,000.00. On December 2,2008 a request for a P.O. was submitted and construction followed thereafter. Change Order: During the repair of the box culvert the contractor encountered unforeseen conditions and utilities as follows: I. The existing soil was over-saturated and unsuitable for back fill. It was necessary to provide new back fill material and haul away the saturated dirt. The cost of new dirt and time to haul away the existing saturated dirt resulted in additional costs of $5,000.00. 2. During construction the contractor encountered an existing concrete roadbed that was not known. To repair the box culvert it was necessary to remove the roadbed and dispose of the concrete. The additional work was completed at a cost of $1 ,900.00 3. During construction the contractor encountered an old signal foundation that was in the path of the proposed storm drain. It was necessary to remove the signal foundation and also the existing sidewalk above the signal foundation. After the work was completed it was necessary to replace the sidewalk. The cost to remove the signal foundation and replace the sidewalk was $1,127.00. Financial Impact: The revised estimated project costs are as follows: TOTAL Original Contract Amount $24,000.00 Contract Change Order No. One $8,027.00 Total Proiect Cost $32.027.00 This change order represents a net increase of 34% of the contract amount. It was No. necessary to provide Class II Base to backfill the storm drain trench. 1-2 Haul Awav Wet Dirt The contractor was directed to haul off the over-saturated soils. Remove Concrete Road Bed 1-3 During construction, the contractor encountered an old existing concrete roadbed. The contractor was directed to remove the roadbed and to dispose of the concrete. Re-Ponr Concrete Sidewalk 1-4 An old signal foundation was discovered in the alignment of the proposed storm drain. The contractor was directed to remove the signal foundation. The removal required sidewalk removal. 1-5 Remove Buried Silmal Foundation & Re-Comnact It was necessary to replace the sidewalk after the removal of the signal foundation. .' ceo No. ONE - Guzman Construction Co.
