1180LUTION ELECTING TO BE SUBJECT TO PUBLIC EMPLOYEES'
MEDIc8' AND HOSPITAL CARE ACT ONLY WITH RESPECT TO MEMBERS OF
THE WATER DEPARTMENT.
WHEREAS, Government Code Section 22850.3 provides that a
contracting agency may elect upon proper application to par-
7 ticipate under the Public Employees' Medical and Hospital
8 Care Act with respect to a recognized employee group only:

WHEREAS,
CITY OF SAN BERNARDINO
, hereinafter
11 referred to as Public Agency is a local agency contracting
with the Public Employees' Retirement System: and

WHEREAS, the Public Agency des~res to obtain for the
members of

WATER DEPARTMENT
IS and annuitants of the agency, the benefit of the Act and to
16 accept the liabilities and obligations of an employer under

17 the Act and Regulations: now, therefore, be it
RESOLVED, that the Public Agency elect, and it does
19 hereby elect, to be subject to the provisions of the Act: and
20 be it further

RESOLVED, that the employer's contribution for each
22 employee who is a member of, and each annuitant who retired
23 from employment which would be covered by the above identi-
24
fied Employee Group (Water Department)
enrolled in PERS
25 health benefits plan shall be as follows:
26
"Sixteen dollars towards the enrollment of each
27 employee and annuitant including family members."

2 And that the contribution shall be in addition to those
3 amount. contributed by the Public Agency for administrative
4 fees and to the Contingency Reserve Fund, and be it further

RESOLVED, that the executive body appoint and direct,
6 and it does hereby appoint and direct, DIRECTOR OF PERSONNEL
7 to file with the Board of Administration of the Public
8 Employees' Retirement System a verified copy of this Resolu-
9 tion, and to perform on behalf of said Public Agency all
functions required of it under the Act and Regulations of the
Board of Administration: and be it further
RESOLVED, that coverage under the Act be effective on

February 1. 1989 .
