Change Order No. 9 approved for installation of hardware for Traffic Signal Interconnect system. ,- CI~Y OF 'SAN BERCARDINO - REQUESTCJoR COUNCIL ACTION STAFF REPORT A permit was required from the San Bernardino County Flood Control District for work involved in widening the Waterman Avenue Bridge over the East Twin Creek Channel. This permit re- quired that the 4 existing pipe gates at their access roads be upgraded to chain link. Since this upgrade was a condition for issuance of the required permit from the Flood Control District, the Contractor was directed to proceed on a time and material basis. The actual costs incurred were $5,492.26. Contract Change Order No. 10 authorizes payment to the Contractor for the actual costs incurred, as provided in Section 9-1.03 of the State Standard Specifications. The estimated quantity of reinforcing steel in the contract documents was 23,760 1bs. This estimate was prepared by the private consulting engineer, based upon a standard of so many pounds of reinforcing steel per cubic yard of concrete. The actual quantity of steel installed was 48,556 Ibs., based upon actual delivery tickets. Section 4-1.03B, of the State Standard Specifications, provides that any increase of quantities in excess of 25% must be authorized by change order. Contract Change Order No. 11 authorizes payment of $12,256.40 to the Contractor for the 18,856 Ibs. of reinforcing steel actually installed, in excess of 125% of the bid quantity. The plans direct that some of the signal heads that are to be removed be salvaged and reinstalled. These heads had 8-inch lenses. During construction, the Caltrans representative noticed that some of these heads were deteriorated due to their long service life, and suggested that new signal heads with 12-inch lenses be installed instead of the salvaged ones. The signal heads to be removed were inventoried, and 15 identified as not suitable for reinstallation. New signal heads were installed at an agreed upon price of $7,365.66. Since this is traffic signal work, the entire cost of Change Order No. 12 will be financed with Federal-Aid Urban Funds. The total cost of Contract Change Orders Nos. 10, 11, and 12 is $25,114.32. There will be a balance of $109,816.93 in contingency funds after the cost of these 3 change orders has been deducted. We recommend that Change Orders Nos. 10, 11 & 12 be approved. 8-25-92 75-0264 c ~ '__..J CONTRACT CHANGE ORDER NO. 10 ROAD Waterman Avenue-Mill to 5th FED NO. TO: E. L.
