STAFF REPORT
Federal Law requires certain registrations and oversight for the
purpose of purchasing and administering controlled substances used
for the humane euthanasia of animals. It is the department's
desire to have a contractual agreement with a local veterinarian
for this purpose.
Dr. Gregory Rex of Animal Medical Hospital has agreed to provide
these services for a nominal fee.

CONTRACT FOR SERVICES
THIS AGREEMENT is entered into this
day of April, 1997, by and between
3 GREGORY A. REX, D.V.M.. doing business as Animal Medical Hospital Association II,
4 (hereinafter referred to as the "CONTRACTOR") and the CITY OF SAN BERNARDINO,
5 (hereinafter referred to as the "CITY"):

A.
WHEREAS, federal law requires certain registrations and oversight for the purpose
8 of purchasing and administering controlled substances used for humanely euthanizing animals; and,

B.
WHEREAS, the CITY desires to have a contractual arrangement with a licensed
10 veterinarian for the purpose of purchasing and administering said controlled substances; and,

c.
WHEREAS, CONTRACTOR possesses the required state and federal registrations
12 necessary to purchase and administer said controlled substances; and,
13
D.
WHEREAS, CITY desires and CONTRACTOR is willing to provide professional
14 services needed for the purchase and administration of said controlled substances.

NOW THEREFORE, the parties hereto agree as follows:

1.
SCOPE OF AGREEMENT.
a.
CONTR.-\CTOR \\.ill obtain and maintain any necessary registration
18 certificates utilizing the address of the CITY'S Animal Shelter as "secondary
19 business" for the purpose of purchasing, administering and storing controlled
20 substances on CITY property by CITY staff;

b.
CITY agrees to pay for all costs of any and all required registration

CITY agrees to pay the costs of all drugs and medical supplies
24 purchased through said registration certificates:

CITY shall keep and maintain records in accordance with the standards and
26 parameters set forth by CONTRACTOR in compliance with federal law;

e. CONTR.-\CTOR shall conduct routine oversight inspections concerning the
purchase, use and storage of said controlled substances on days and times to be mutually

COMPENSATION. CONTRACTOR will be paid $50.00 per month by the CITY

3 for the services provided under SCOPE OF AGREEMENT. CONTRACTOR will provide to
4 CITY a monthly invoice in said amount.
3. TERM. This contract shall take effect upon execution by both parties.
4. TERMINATION OF CONTRACT. The CITY or CONTRACTOR has the right
to terminate this contract for services for any reason by the giving of fourteen days wntten notice.
5. NOTICES. All notices herein required shall be made to the parties as follows:
