Staff Report
Resolution of the Mayor and Common Council of the City of San Bernardino to amend Sucaba Resolution No.
2006-309 approved on September 5, 2006.
Backl!round:
On September 5, 2006 Mayor and Common Council approved the Resolution to authorize 'an increase in
Purchase Order No. 206413 to Sucaba Company, Inc. and to request a Purchase Order to be issued in the
amount of$42,000 for FY 2006-07. The request for FY 2006-07 was written in the staff report but inadvertently
left out in the resolution.
Financial Impact:
With both the Risk Management and Workers' Compensation divisions utilizing the services ofSucaba, the
Human Resources Department is requesting a purchase order be issued in the amount of $42,000 for FY 2006-
07.
Recommendation:
I. Adopt Resolution.

BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL OF THE CITY 0
SAN BERNARDINO AS FOLLOWS:
SECTION 1: Resolution 2006-309 is hereby amended to authorize the Director 0
Finance to increase Purchase Order No. 207455 to $42,000 for FY 2006-07.
SECTION 2: The authorization to execute the above-referenced Purchase Order i
rescinded ifnot issued within sixty (60) days of the passage of this resolution.
