Subject:
Authorize the Director of Development Services to execute contract Change Order No. One to a contract with
J.D.C., Inc. for the Construction of ADA Access Ramps and Sidewalk at Gilbert Street (SS-B 05106), per Plan
No. 11567.
Background:
On August 15, 2005, the Mayor and Common Council allocated funds, as part of the FY 2005/06 Capital
Improvement Program (CIP), for the Construction of ADA Access Ramps and Sidewalks at Gilbert Street,
2005/06, (SS-B).
On December 19, 2005, the Mayor and Common Council approved a Resolution to award a contract to J.D.C.,
Inc., in the amount of $68,925.00 for the "Construction of ADA Access Ramps and Sidewalk at Gilbert Street"
(SS-B 05/06), per Plan No. 11567.
During construction, it became apparent that specific additional tasks (as described in the attached Change Order
No. One) were necessary to properly construct this project.
Item I-I "Adjust Manholes to Grade." During construction an unmarked manhole became visible. It became
necessary to adjust/raise this manhole cover to grade.
Item 1-2 "Remove Asphalt." It was necessary to revise a 7-ft. wide, rather than a 6-ft. wide X I 89-ft. long strip
of asphalt to allow the street cross-fall to match to new lip of the gutter.
Item 1-3 "Remove extra thick Asphalt Concrete." When cutting (grading) for the new curb and gutter, the
contractor found 12-inch to 16-inch thick asphalt pavement that needed to be removed prior to pouring the curb
and gutter. The asphalt thickness was anticipated to be 3-4 inches; however, the thickness was found to vary
between 12 inches and 16 inches. As a result, the contractor was required to perform extra work for which they
are entitled to additional compensation.
Item 1-4 "Saw Cut Asphalt Concrete." During the removal of the asphalt concrete, the existing edge of the
pavement was not straight and clean. Saw cutting the asphalt concrete was necessary to create a straight and
clean line to construct against.
The cost of these changes was determined by tracking time and materials for the work.
Financial Impact:
The total cost of Change Order No. One is $5,492.05. A sufficient contingency funding for this change order is
available in Account No. 129-367-5504-7202 "Construction of ADA Access Ramps and Sidewalk at Gilbert
Street (SS-B 05/06)."
Recommendation:
Authorize the Director of Development Services to execute Contract Change Order No. One in the amount of
$5,492.05 to the contract with J.D.C., Inc. for Construction of ADA Access Ramps and Sidewalk at Gilbert
Street (SS-B 05/06), per Plan No. 11567.

JUSTIFICATION:
Item Adiust Sewer Manhole to Grade
1-1 During construction, a manhole was discovered. This manhole was buried and not visible
No. prior to the start of construction. It was necessary to raise the manhole to grade.
Item Remove 189' Xl' of Asphalt
No. 1-2 It was necessary to remove a 7' wide by 189' long strip of asphalt pavement to allow the
street crossfall to match the new lip of gutter.
Item Remove 16" A.e.
No. 1-3 When making grade for the curb and gutter asphalt, which was varied between 12" and
16" thick was encountered, it was necessary to remove this asphalt.
Item Saw cut A.C.
1-4 During the removal of asphalt, in order to achieve a clean vertical edge, it is necessary to
