STAFF REPORT
The DENTICARE (Mission Dental) Plan was first made available
for purchase by employees in 1984. Since that date, the
monthly rates have not been increased and there will be no
change in the current monthly rates until August 1, 1990. The
current rates are:

LOW OPTION

Employee + Family

On August 1, 1990 the Low option rates will be increased to:

Employee + Family
The High Option rates will not change.
The attached Resolution extends the term of our agreement
with DENTICARE (Mission Dental) through July 31, 1991. This
date will coincide with our PERS Health Benefit Plans renewal
date.

Staff intended to bring this renewal before Council prior to
the expiration of the last agreement, however, DENTICARE
staff did not provide the City with the revised agreement
until March 28, 1990. Employees and covered dependents will
not experience any change in service due to this delay.
The Low Option rates need to be increased in August in order
for DENTICARE to meet the higher level of capitation to their
providers. This enables DENTICARE to maintain a stable panel
of quality providers to render treatment to city employees
and their families.

2 EXECUTION OF AN AMENDMENT TO AGREEMENT WITH DENTICARE
(MISSION DENTAL) PLAN EXTENDING THE TERM FOR AN ADDITIONAL

SECTION 1. The Mayor of the City of San Bernardino is
hereby authorized and directed to execute on behalf of said
city an Amendment to Agreement with Denticare (Mission
Dental) Health Plan extending the term for an additional
nineteen month period, effective January 1, 1990 through July
31, 1991, relating to a Dental Plan for employees and
dependents, and to reflect a rate increase for the Low Option
portion of the plan, a copy of which is attached hereto and
incorporated herein as Exhibit "A".

EXHIBIT A
FOURTH AMENDMENT TO AGREEMENT
(General Dentistry Plan with Orthodontic Benefit)
THIS FOURTH AMENDMENT TO AGREEMENT is entered into effective
January 1, 1990, between DENTICARE (MISSION DENTAL) HEALTH
PLAN, herein-after referred to as a "Plan", and the CITY OF
SAN BERNARDINO, hereinafter referred to as a "Group". Plan
and Group agree as follows:
1. Recitals.
(a) Plan and Group entered into an agreement on
January 10, 1984, effective January 1, 1984, relating to a
General Dentistry Plan with Orthodontic benefits.
(b) The parties now desire to amend the agreement
to add nineteen months to the agreement and show a rate
change effective August 1, 1990.
2. Term. The term of the agreement shall be extended
for an additional period of nineteen months commencing on
January 1, 1990, and terminating on July 31, 1991.
3. Other Provisions. Effective August 1, 1990 the
Low option monthly rates will be changed to:
Employee only $ 8.00
Employee + one $13.00
Employee + family $18.00
All other terms, conditions and covenants of the agreement
shall remain the same and be in full force and effect.
