Synopsis of Previous Council action:
12/7/81 -- Council approves action permitting dependent spouse of deceased
City employee to participate at his/her own expense in the City's Blue
Cross Medical Insurance. Program for a period not to exceed 3 years, foll-
owing the death of the employee.

STAFF REPORT
In December 1981, the Council adopted a policy of allowing
the surviving spouse of an active city employee who became
deceased to continue under the self insurance medical program
of the City for a period of three years. This item has been
re-discussed recently by the Council Personnel Committee and
it was recommended that the policy be extended for an
additional two-year period making a total of five years or
until the surviving spouse attains the age of 65, is remar-
ried or is covered under another health plan. The attached
resolution will 6ffect such a policy. It is anticipated that
this will not add any cost to the City's present self in-
surance program.

RESOLUTION OF THE CITY OF SAN BERNARDINO AUTHORIZING
EXTENSION OF THE SELF INSURED MEDICAL PLAN CONTINUATION PRO-
GRAM FOR THE SURVIVING SPOUSE OF AN ACTIVE EMPLOYEE.
BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL OF THE
CITY OF SAN BERNARDINO AS FOLLOWS:
SECTION 1.
Whereas, in December 1981, the City
Council adopted a policy of self insured medical program
continuance for a surviving spouse of an active employee for
a period of three years; and
SECTION 2.
Whereas, the City of San Bernardino is
currently providing this option of plan continuance on the
City's self-insured plan for the bereaved spouse of an
active employee, if the spouse is enrolled in the self
insured plan at the time of the active employe~s death and
pays
the appropriate premium
during the continuation
coverage period: and
SEC'rION 3. Whereas, the City of San Bernardino bas
previously established a three-year limit for such a plan:
and
SECTION 4.
Whereas, the City's Council Personnel
Committee recommended that the policy be extended by two
years, for a total of five years, at their meeting held
Harch 24, 1986.
SECTION 5. Now, therefore be it resolved that sllch a
continuance program be extended for an additional two-year
period for a total of five years, or the attainment of age
65, remarriage or coverage under another health plan,
whichever occurs first.
