CITY OF SAN BERNARDINO - REQUEST FOR COUNCIL ACTION
STAFF REPORT
During excavation for the storm drain, the Contractor encountered an
abandoned reinforced concrete vault. This structure was not shown on the
plans, since it was not of record and its existence was unknown. Removal of
this abandoned vault was required in order for the storm drain and appurten-
ant facilities to be installed in accordance with the plans and specifications.
Two water mains were also broken by the Contractor during excavation. These
mains were not shown on the plans since the serving utility was not aware of
their existence. The Contractor was compelled to shut down his entire crew,
consisting of 6 personnel and 4 pieces of equipment, for a hour.
Payment to the Contractor will be made on a time and materials basis,
for removal of the vault and shut down time.
The total estimated project cost, based upon the actual low bid price,
contained an mount of $28,918 for contingencies, which will be used to finance
the cost of this change order.
We recommend Change Order No.1 be approved.
