The franchise permits Care Ambulette to operate two dialysis transportation vehicles within the city, At their meeting of June 11, 1985 the Bureau of Franchises recommended that a second dialysis transportation provider, Southwest Medical Transport, Inc, located at 1802 East Cedar, Suite A, Ontario, California be granted a franchise to operate three dialysis transportation vehicles. The Bureau of Franchises based its recommendation on interviews with Dr. Kavalich who is Director of the San Bernardino Valley Dialysis Center as well as other staff members of the dialysis center. The decision to recommend a second franchise for dialysis transportation was primarily based on what would be most beneficial to the dialysis patients, Two dialysis transporation franchises would provide patients with a choice of providers. Consideration was also given to the competitive aspect of having two dialysis transportation providers and the positive effect this would have re- garding service, Southwest Medical Transport, Inc. owner Ms, Nancy Olson has complied with all requirements set forth in Municipal Code Chapter 5.76 relative to dialysis transportation. This includes proof of insurance in the amount of $1,500,000,00 and payment of the franchise permit filing fee, Vehicles to be used under the proposed franchise were inspected on June 27, 1985 and found to meet all require- ments with respect to the transportation of dialysis patients, Rates for dialysis transportation charged by Southwest Medical Transport, Inc, will be the rates established by the Medi-cal Program of the State of California, 15-0264
