Amendment to the Professional Services Agreement
with Jones & Mayer for the completion of the City's
Charter Implementation efforts, increasing the total
amount of the agreement by $50,000, and authorizing

Recommendation: Adopt Resolution No. 2018-162 of the Mayor and City

San Bernardino.
74. Seccombe Lake Park and Perris Hill Park Improvements Award of
Construction Contract to California Landscape & Design, Inc.
Recommendation: Adopt Resolution No. 2018-163 of the Mayor and City
Council of the City of San Bernardino, California,
approving a construction contract with California
Landscape & Design, Inc. in the amount of $981,000
and authorizing a construction contingency in the
amount of $98,000, for a total contract amount of
$1,099,000; waiving minor irregularities in bid
documents; authorizing the City Manager or designee
to sign all necessary documents; authorizing the City
Manager to expend the contingency fund, if
necessary, to complete the project; and authorizing
the Finance Director to amend the 2017/18 budget to
appropriate $369,000 from Parkland Fund to
complete the full funding of the Seccombe Lake Park
and Perris Hill Park Improvements.

Recommendation: Adopt Resolution No. 2018 -16& -of the Mayor and City
Council of the City of San Bernardino, California,
approving a construction contract with Sierra Pacific
Electrical of Jurupa Valley in the amount of
$1,403,390 and authorizing a construction
contingency in the amount $140,339, for a total
contract amount of $1,543,729; authorizing the City
Manager or designee to sign all necessary
documents; authorizing the City Manager to expend
the contingency fund, if necessary, to complete the
project per Special Provisions No. 13193, and
authorizing the Finance Director to amend FY
2018/19 budget to appropriate $244,000 from Gas
Tax to complete the full funding of the Knocked -Down
Street Light Pole Replacement Services.

Agreement with the San Bernardino County
Transportation Authority (SBCTA) for the
Development of the Mount Vernon Avenue Grade
Separation over the BNSF Railway Intermodal Yard

City of San Bernardino, Active Transportation Program (ATP) Cycle 4 - Safe
Route to School (SR2S) Grant Application
Recommendation: Adopt Resolution No. 2018-165 of the Mayor and City
Council of the City of San Bernardino, California,
approving a list of projects to apply for the Active
Transportation Program (ATP) Cycle 4 - Safe Route
to School (SR2S) grant application; authorizing the
execution of a co-operative agreement with the City of
Highland and the City of Rialto to submit a combined
ATP application, and authorizing the City Manager or
designee to sign all necessary documents.

Award of a Contract for Annual Traffic Signal Video & Loop Detector
Installation Services per Special Provisions No. 13209 to Siemens Industry,
Inc.
Recommendation: Adopt Resolution No. 2018-166 of the Mayor and City
Council of the City of San Bernardino, California,
approving a contract between the City of San
Bernardino and Siemens Industry, Inc. of Riverside,
California, for traffic signal video and loop detector
installation services and issuance of an Annual
Purchase Order not to exceed $359,955 with four (4)
one-year extensions at the City's option to Siemens
for traffic signal video and loop detector installation
services per Special Provisions No. 13209, and
authorizing the Finance Director to amend FY
2018/19 budget to appropriate $59,955 from Gas Tax
to complete the full funding of the Annual Traffic
Signal Video & Loop Detector Installation Services.

Works
Recommendation: Adopt Resolution No. 2018-167 of the Mayor and City
Council of the City of San Bernardino, California,
approving a construction contract with S&H Civil
Works, in the amount of $1,347,900 and authorizing a
construction contingency in the amount of $134,790,
for a total contract amount of $1,482,690; authorizing
the Finance Director to amend FY 2017/18 budget to
allocate Measure I Funds to the project, authorizing
the City Manager or designee to sign all necessary
documents; and authorizing the City Manager to
expend the contingency fund, if necessary, to
complete the project.

Recommendation:
Adopt Resolution No. 2018-169 of the Mayor and City

authorizing the execution of a Vendor Services
Agreement between Fox Occupational Medical
Center and the City of San Bernardino for pre-
employment physicals and evaluations.
2. Vendor Services Agreement between The Counseling Team International
(TCTI) and the City of San Bernardino for an Employee Assistance Program
(EAP)
Recommendation: Adopt Resolution No. 2018-170 of the Mayor and City

authorizing the execution of a Vendor Services

Recommendation: Adopt Resolution No. 2018-171 of the Mayor and City
Council of the City of San Bernardino, California,
establishing a set vehicle reimbursement allowance
for eligible City employees and elected officials, and
repealing Resolution No. 2006-428.

✓ Counties Excess Insurance Authority (CSAC-EIA) insurance pool for
excess workers' compensation insurance and purchase stand-alone
commercial coverage for excess liability insurance with Aon Risk Services
Recommendation: Adopt Resolution No. 2018-172 of the Mayor and City
Council of the City of San Bernardino, California,
authorizing the City Manager to enter into California
State Association of Counties Excess Insurance

Services Agreement with RP Landscape & Irrigation
for landscape maintenance at the City's North,
Central, and South park sites.
�6. Issuance of a Purchase Order to BSN Sports for Tennis Court Renovations
at Perris Hill Park and amend the FY 2017/18 Operating Budget to include
estimated revenues and appropriation of $314,200 of grant funds
Recommendation: Adopt Resolution No. 2018-174 the Mayor and City
Council of the City of San Bernardino, California,
authorizing the Director of Finance to amend the FY
2017/18 budget to reflect additional revenues in the
amount of $314,200 and issue a purchase order for
$46,151.51 to BSN Sports for the purchase of
materials and supplies for tennis court renovations at
Perris Hill Park.
2017-2018 Retired and Senior Volunteer Program (RSVP) — 3rd Year of a 3 -
Year Grant Award
Recommendation:
Adopt Resolution No. 2018-175 of the Mayor and City
Council of the City of San Bernardino, California,
ratifying the submission of a grant application for year
three of the National and Community Service's
Retired and Senior Volunteer Program, accepting and
appropriating the grant funds in the amount of
$47,310 and allocating matching funds for the
program for the period of April 1, 2018 through March
31, 2019.

Playground Tile Surface at Perris Hill Park
Recommendation: Adopt Resolution No. 2018-176 of the Mayor and City
Council of the City of San Bernardino, California,
authorizing the City Manager to execute a Vendor
Services Agreement with SofSurfaces, Inc. for
purchase and installation of rubber playground tile
surface at Perris Hill Park.

Recommendation: Adopt Resolution No. 2018-186 of the Mayor and City

authorizing the City Manager to execute a Vendor
Services Agreement with PlayPower LT Farmington,
Inc. for removal and replacement of the playground
equipment located at the eastern portion of Perris Hill
Park.
3�. Approve Final Tract Map 16502 Beazer Homes Holding Corporation
Recommendation: Adopt Resolution No. 2018-178 of the Mayor and City
Council of the City of San Bernardino, California,
approving Final Map for Tract No. 16502 (Subdivision
03-10) involving the subdivision of a parcel containing
approximately 14.4 acres into forty (40) separate
parcels (38 single-family residential lots and 2
drainage basin lots) located northwest of the
intersection of W. Ohio Avenue and N. Magnolia
Avenue within Residential Low (RL) Zone, accepting
the public dedications as set forth on said map; and
authorizing execution of the standard form of
agreement for the subdivision improvements.
Authorize Agreement with Phoenix Group Information Systems for Parking
Citation Processing Services
Recommendation: Adopt Resolution No. 2018-179 of the Mayor and City
Council of the City of San Bernardino, California,
authorizing the City Manager to execute an
Agreement between the City of San Bernardino and
Phoenix Group Information Systems for parking
citation processing for FY2018/19 with the option for
three one-year renewals.

Recommendation: 1) Accept for final reading and adopt Ordinance No.
MC -1495, approving General Plan Amendment
16-07 and Development Code Amendment
(Zoning Map Amendment) 16-08 to change the
General Plan Land Use Designation from Single -
Family Residential to Commercial and the Zoning
District Classification from Residential Suburban
(RS) to Commercial General (CG -1) of one (1)
parcel containing a total of approximately 10,000
square feet; and
2) Adopt Resolution No. 2018-158 of the Mayor and
City Council of the City of San Bernardino,
California, adopting the Mitigated Negative
Declaration, approving Conditional Use Permit 16-
17 to allow a service station, a convenience store
and an express drive-thru carwash facility on two

approving the City of San Bernardino's Operating
Budget and Capital Improvement Program (CIP) for
FY 2018/19, establishing the City's Appropriations
Limit Consistent with Article XIII of the California State
Constitution, and FY 2018/19 Salary Schedule.
J. Calling of a General Municipal Run -Off Election and Requesting the San
Bernardino County Registrar of Voters to Conduct Said Election
Recommendation:
Adopt Resolution No. 2018-187 of the Mayor and City
Council of the City of San Bernardino, California,
calling and giving notice of the holding of a General
Municipal Run -Off Election to be held on Tuesday,
November 6, 2018, requesting the Board of
Supervisors of the County of San Bernardino to
consolidate said Election with the November 6, 2018
Statewide General Election and permit the San
Bernardino County Registrar of Voters to render
Election services to the City of San Bernardino for
said Election, and authorizing the City Manager to pay
the invoice for the costs of the Election.

NOTICE: Any member of the public may address this meeting of the Mayor and City Council and
the Mayor and City Council Acting as the Successor Agency to the Redevelopment Agency on
any item appearing on the agenda by approaching the microphone in the Council Chamber when
the item about which the member desires to speak is called and by asking to be recognized.
Any member of the public desiring to speak to the Mayor and City Council and the Mayor and
City Council Acting as the Successor Agency to the Redevelopment Agency concerning any
matter not on the agenda but which is within the subject matter jurisdiction of the Mayor and City
Council and the Mayor and City Council Acting as the Successor Agency to the Redevelopment
Agency may address the body at the end of the meeting, during the period reserved for public
comments. Said total period for public comments shall not exceed 60 minutes, unless
such time limit is extended by the Mayor and City Council and the Mayor and City
Council Acting as the Successor Agency to the Redevelopment Agency. A three minute

next agenda of the Mayor and City Council and the Mayor and City Council Acting as the
Successor Agency to the Redevelopment Agency. However, no other action shall be taken nor
discussion held by the Mayor and City Council and the Mayor and City Council Acting as the
Successor Agency to the Redevelopment Agency on any item which does not appear on the
agenda unless the action is otherwise authorized in accordance with the provisions of
subdivision (b) of Section 54954.2 of the Government Code.
Public comments will not be received on any item on the agenda when a public hearing has
