I -e q S e See SAN BERNARDINO FINE ARTS COMMISSION , +-44 Ched- 2017-18 GRANTS FINAL REPORT Dear Grantees: Your organization received a grant from the City of San Bernardino Fine Arts Commission to be used between July 1, 2017 and June 30, 2018. Although the Fine Arts Commission has been dissolved, it was very important to the former Commissioners to work to ensure the continued funding of Arts Organizations within our community. One of their final requests as a Commission was that the City Clerk's Office follow through with the evaluation process and forward the results to the new Fine Arts and Historical Preservation Commission, in the hopes that the new commission would see the effectiveness of awarding grants to arts organizations and continue to lobby for and give funding to such Arts Organizations in San Bernardino. Please fill in the form below. Return by Monday, July 30, 2018 to: City of San Bernardino Office of the City Clerk — Attention: Diane Grant 290 N. "D" St., San Bernardino, CA 92401 Please Note: Many of the former Fine Arts Commissioners expressed an interest in serving on the new commission and if appointed will be looking for these evaluations to help in creating any processes the new commission uses to fund grants for organizations if they receive funding through the general budget to do so. The future availability of funding and the processes used to choose the organizations to be funded may depend on the ability of the new commission to access the information you provide. 1.
