AUTHORIZATION FOR APPLICATION AND
ADMINISTRATION OF A GRANT IN THE
AMOUNT OF $ 20,000 FROM CATHOLIC
HEALTHCARE WEST FOR A SENIOR
NUTRITION VAN.
Catholic Healthcare West (CHW), the operator of St. Bernardine Medical Center, provides
community grants. The community grants are intended to address a wide range of community
needs in the areas they serve. CHW is committed to promoting universal access to needed
healthcare services and addressing the underlying socio-economic causes of illnesses. Through
its community grants program, CHW seeks to "partner" with other community groups to
improve the health status and the quality of life of the communities it serves.
The CHW Community Grants Program is designed to be a funding resource which supports and
enhances the advocacy, social justice and healthier communities efforts of its divisions, regions
and sponsoring congregations.
CHW's Community Grants Program seeks to support organizations whose efforts embody its
five core values: the dignity of each person, excellence, justice, stewardship and in particular,
collaboration.
The Parks, Recreation and Community Services Department has program needs that are highly
suitable to the CHW Grants. The department has been desirous of acquiring a Senior Nutrition
Cargo Van to replace the one destroyed by an engine fire two years ago. Acquisition of this van
could save the Nutrition Program about $ 4,000 a year which could go back into raw foods and
other direct services. Grant proposal is for $ 20,000.

What is the mission of your organization?
To develop programs and operate facilities that enhance social interaction and provides
opportunities for all residents to share their heritage, values, abilities and ambitions.
To provide for all citizens a variety of leisure and social service opportunities that
create new interests, develop new skills, and provide for basic needs in order to develop
a citizenry that is healthy and creative.
Briefly describe the project outlined in this proposal. Who are its beneficiaries? How will CHW
funds be used?
The Senior Nutrition program sponsored by the City of San Bernardino provides senior
citizens 60 years and older a nutritious lunch time meal 254 days per year. Prepared
meals are delivered from two preparation kitchens to six satellite sites. During
Fiscal Year 1995/96 over 90,000 meals were served. Currently, the program requires a
transport vehicle to maintain its routes. The routes are to 6 satellite nutrition sites
that receive daily delivery of prepared senior meals.
Describe the community you serve (i.e. size, location, socioeconomics, ethnicity, etc.] and how this
project responds to a need identified by its residents.
The City of San Bernardino is located approximately 60 miles east of the City of Los
Angeles at the base of the San Bernardino mountains. According to the 1990 Census
Data, there were 163,150 residents of the City of San Bernardino, 64,561 are non-white.
The Older American's Act of 1965 requires the area agency on aging to target services
and identify individuals eligible for assistance with special emphasis on older
individuals with greatest social and economic need with special attention to minority
and low-income seniors. San Bernardino City has 21,114 citizens 60 years and older
according to the 1990 Census.

1996 COMMUNITY GRANTS PROGRAM
I. ORGANIZATION
�.. The Senior Nutrition Program is a joint venture between the Government sector and its
citizens. The Federal Government's Older American's Act of 1965, as amended,
provides that State and Local Governments form area agencies to accept and deliver
funds that are directed to senior citizens programs geared to attack social ills affecting
Americans 60 years of age and older.
All funds are made available through a formula of needs assessment. Consideration is
given for an area's location, rural/urban profile and its economic, racial, ethnic
population base. Funding is not meant to completely finance a given program, but to
provide a foundation to a program, to providE basic services. Unforeseen expenses such
as equipment replacement may not be funded. Funded local agencies must be able to
establish their credibility as it relates to project goals. The City of San Bernardino is a
funded service provider. The City of San Bernardino, as a direct service provider to
Senior Nutrition needs, provided nearly 90,000 senior residents such meals in 1995-96.
These meals are provided to 8 congregate (on site) meal sites, and prepared at 2
preparation kitchens operated by the City of San Bernardino.
B. To provide congregate (on site) nutrition services that assist older individuals to live
independently by promoting better health through improved nutrition and reduced
isolation through a program coordinated with other supportive services.
C. The City of San Bernardino Parks, Recreation and Community Services Department is
the largest municipal recreation agency in the Greater Inland Empire. In addition to the
Senior Nutrition Program, the department operates a myriad of recreational, social and
cultural programs for all ages of the population. Additional services to seniors include:
• Information and Referrals to Other Senior Supportive Services
• Phone ALERT (P.A.L.) for Senior Citizens
• Retired Senior Volunteer Program (R.S.V.P.)
• Health Screening Clinics
• AQewise Outreach Programs (for Seniors with Mental Disorders)
• Free Income Tax Preparation
• Arthritis Exercise Programs
• And Many More
The Parks. Recreation and Community Services Department also operated two (2)
dedicated Senior Service Centers: Perris Hill Senior Center, 780 East 21st Street, San
Bernardino and Downtown Senior Center, 600 West Fifth Street, San Bernardino.

II. PROJECT
A. The Senior Nutrition Program of the City of San Bernardino will servc �_Ju,uu0 meals
during Fiscal Year 1996/97. These n �-ais will be prepared and served at eight (8)
C,-•ngregate sites. Meals are delivered to satellite congregate sits by two (2) cargo vans.
The geographic area S.,rvea o� this contractor covers the communities of San Bernardino
and Highland. The clientele for congregate nutrition consists of:
a. Seniors 60 years and over.
b. The spouse of any person 60 years and over.
C. Handicapped or disabled individuals who reside in a housing facility occupied
primarily by the elderly at which congregate nutrition services as provided.
d. A handicapped or disabled child who lives with an elderly individual.
e. Any volunteer regardless of age who volunteers his/her time at a meal preparation
site at least eight (8) hours per month.
Listed below are the City of San Bernardino's congregate feeding sites and their
affiliation:
• Downtown Senior Center (Preparation Site) 600 West 5th Street
• Perris Hill Senior Center (Preparation Site) 780 East 21st Street
• Hernandez Community Center (City Owned & Operated) 222 North Lugo Avenue
• Public Enterprise Building (City Owned & Operated) 1505 West Highland
• Lytle Creek Community Center (City Owned & Operated) 380 South "K" Street
• Nicholson Community Center (City Owned & Operated) 2750 West 2nd Street
• Highland Senior Center (Private - Non-profit) 3102 East Highland
• Adult Day Health Care (Operated by Community Hospital) 3000 East Highland
B. By U.S.D.A. regulations, all meals delivered to congregate sites must be delivered within
one (1) hour of the designated service time and the meals service time must be scheduled
during a one (1) hour time frame between 11:30 a.m. - 1:30 p.m. For example, a site
with a meal serving time of 12:00 to 1:00 p.m., must receive its meals between 11:00
a.m. and 12:00 p.m. The need for two delivery vehicles is essential to meet this
schedule. Over the past two (2) years, the Nutrition Program has been renting cargo
vans at an astronomical cost. Vehicle operation expense for a 1990 Ford Econoline Van
currently owned by the Senior Nutrition Program is approximately $ 3,500 per year.
The cost to rent and operate the second required van was over $ 7,000 for the 1995-96

period, double the budgeted amount. Through the purchase of a delivery vehicle the
Senior Nutrition Program will save a minimum of$ 4,000 that can be applied to cover
a 1996-97 budget cut of 5.2 percent.
C. The City of San Bemar_': .,) Senior Nutrition Program has incorporated %vithin its
Objectives the following:
Proviu<, congregate meals to at least 2,000 un-duplicated seniors of which 150 are
minority low-income seniors.
According to the 1990 Census Data, San Bernardino had over 21,114 seniors, 60 years
and older, of tvh;r_h 2,054 were below the poverty line. In addition a number of those
identified seniors are diseinfrancished tbrough language barriers and lack
transportation, both public and private. Without the delivery of senior nutrition meals
to the cities existing, satellite nutrition sites, 100-170 seniors would not eat a nutritious
meal daily.
All of our satellite sites are established in low-income minority areas making them truly
accessible to the targeted resident population.
D. The Senior Nutrition Program provides single source nutrition to many seniors 60 years
and older participating in the program 6 days per week. The Senior Nutrition Program
is supported by over fifty (50) volunteers who contributed over 10,000 volunteer hours
during Fiscal Year 1995-96. There are no other nutrition services provided for seniors
other than the City's Senior Nutrition Program.
E. The City of San Bernardino's Senior Nutrition Program is a cooperative effort involving
key community service agencies providing senior services.
The County of San Bernardino's Department of Aging and Adult Services is the primary
funding and support agency to the City's Senior Nutrition Programs. In addition to
funding a major portion of nutrition meal services at City owned and operated Senior
Nutrition sites, the County provides nutrition education classes to seniors, senior
employment in support of senior program, Meals On Wheels to senior shut-ins and
forums that provide seniors an opportunity to access information and input their concerns
thus empowering the collective group.
The Senior Nutrition Program is the primary program established at the City's two
dedicated Senior Service Centers. On-site funding exposes seniors to a variety of service
providers who contract with the City of San Bernardino to utilize space at our senior
facilities.
In addition to City owned and operated facilities, other agencies, businesses and senior
exclusive programs contract with the City to provide meals to their respective facilities.

F. The work plan established for the City of San Bernardino contract with its Fiscal Year
1996-97 primary funding agency is a joint-effort between the County of San Bernardino
Department of Aging and Adult Services, "DAAS", (the over-site agency) and the City
of San Bernardino (the contactor). The work plan specifically details performance
objectives expected of the contractor.
Listed i.e:e ure those criteria that the City of San Bernardino Parks, Recreation and
Community Services Department will fulfill and document to the County of San
Bernardino to evaluate the success of this project.
1. To serve 35 meals per day.
2. To maintain 6 satellite congregate nutrition sites; and 2 preparation kitchens.
3. To deliver 100 - 170 Senior Nutrition meals to 6 congregate sites daily, Monday
through Friday, 254 days per year.
The use of two (2) delivery vehicles is essential to accomplish these objectives.
G. Oscar J. Perrier, Senior Recreation Supervisor, has been employed by the City of San
Bernardino for twenty-five years. For the past ten years, Mr. Perrier has been the
Administrative Supervisor responsible for the Senior Nutrition Program's request for
funding and budget over-site and on-going operations.
