Monday, January 9, 2006 2006-1 Resolution granting an extension of the Cable Franchises with Adelphia Communications, Charter Communications, and Mountain Shadows Cable. 2006-2 Resolution conditionally authorizing the assignment of a Cable Television Franchise by Adelphia Cablevision of Inland Empire, LLC to Time Warner NY Cable, LLC, an indirect subsidiary of Time Warner Cable Inc. 2006-3 Resolution conditionally authorizing (1) a transfer of control of the Cable Television Franchise from Adelphia Communications Corporation to Comcast Cable Holdings, LLC; (2) an assignment of the Cable Holdings, LLC, to CAC Exchange I, LLC; and (3) a transfer of control of CAC Exchange 1, LLC from Comcast Corporation to Time Warner Cable Inc. 2006-4 Resolution approving the destruction of certain records no longer required to be maintained by the San Bernardino City Attorney's Office. 2006-5 Resolution approving the Final Map for Tract Map No. 16795, located between Belmont and Ohio Avenue, west of the vacated Chestnut Avenue in the RL, Residential Low land use district, accepting the public dedications as set forth on said map; and authorizing execution of the standard form of agreement for the improvements in said subdivision, with time for performance specified. 2006-6 Resolution authorizing the Mayor to execute a Standard Contract with the County of San Bernardino, the City of Redlands, the City of Highland and Inland Valley Development Agency (IVDA) to construct culverts at Alabama Street and City Creek. 2006-7 Resolution authorizing the City of San Bernardino to renew the Lease Agreement with the San Bernardino International Airport Authority to provide aircraft rescue and fire fighting services at the San Bernardino International Airport. 2006-8 Resolution amending Resolution No. 6429, entitled in part "A Resolution.... establishing a basic compensation plan for officers and/or employees of the Police and Fire Departments of the City. 2006-9 Resolution authorizing the execution of a purchase order to Fairview Ford for the purchase of a 2006 F-250 4WD crew cab pick up truck, to be utilized by the Police Department. 1 Monday, January 9, 2006 (cont.) 2006-10 Resolution making application to the Local Agency Formation Commission of the County of San Bernardino for annexation to the City of San Bernardino of unincorporated territory, generally located north of Highland Avenue, east of Golden Avenue, to be known as Annexation No. 359. 2006-11 Resolution authorizing the Mayor or her designee to execute Second Amendment to extend the Agreement for Use of Facilities (Adult School) between the City of San Bernardino and the San Bernardino City Unified School District. 2006-12 Resolution authorizing the San Bernardino City Fire Department to apply for and administer the 2007 Office of Traffic Safety (OTS) grant to be submitted to the State of California Business, Transportation and Housing Agency in the amount of $116,000. (Stantec) to provide sewer line capacity evaluations, including field flow tests and to make recommendations for upgrades and improvements of sewer lines at various locations in the City, SW04-21, SW04- 36, SW05-01, SW05-02, SW05-05, SW05-06 and SW05-07, (City funded projects), and repealing Resolution No. 2005-384. 2006-32 Resolution approving the Final Map for Tract Map No. 16547, located at the northeast corner of Pine and Ohio Avenues in the RL, Residential low land use district, accepting the public dedications as set forth on said map; and authorizing execution of the Standard Form of Agreement for the improvements in said subdivision, with time for performance specified. 2006-33 Resolution authorizing the execution of a purchase order to Crest Chevrolet for the purchase of two 2006 3/4 ton standard cab chassis complete with Supreme Truck Services bodies, to be utilized by the Police Department. 2006-34 Resolution authorizing the execution of Amendment No. 1 to the Agreement for Consultant Services in collaboration with District for the CAPS Program between the City and the San Bernardino Unified School District. 2006-35 Resolution adopting an Enforcement Response Plan establishing appropriate responses and penalties to non-compliant users who discharge non-domestic wastewater to the San Bernardino Water Reclamation Plan. (Repealed by Resolution No. 2006-57). 2006-36 Resolution approving an Amendment to the 2005 Meadowbrook Single - Family Residential Development Agreement by and between the Redevelopment Agency of the City of San Bernardino and Meadowbrook Park Homes, Inc. (Central City East Redevelopment Project Area). 2006-37 Resolution ratifying submittal of the application for Waste Tire Enforcement Grant funds from the California Integrated Waste Management Board. Tuesday, February 21, 2006 2006-42 Resolution approving the destruction of certain records no longer required to be maintained by the San Bernardino City Attorney" Office. 2006-43 Resolution amending Professional Services Agreement between Mundell, Odlum & Haws and the City of San Bernardino regarding representation related to conducting a personnel investigation. 2006-44 Resolution reciting the facts of the General Municipal Election held in said city on the 7' day of February 2006, declaring the results thereof and setting forth such other matters as are provided by law. 2006-45 Resolution making application to the Local Agency Formation Commission of the County of San Bernardino for the expansion of the Sphere of Influence of the City of San Bernardino and annexation of unincorporated territory located in the Arrowhead Springs area, generally northerly and northeasterly of State Highway 18 and Old Waterman Canyon Road. 2006-46 Resolution making application to the Local Agency Formation Commission for providing City sewer services to property within unincorporated territory located at 1959 Porter Street. 5 Tuesday, February 21, 2006 (cont.) 2006-47 Resolution making application to the Local Agency Formation Commission for providing City sewer services to property within unincorporated territory located at 3752 Duffy Street. 2006-48 Resolution making application to the Local Agency Formation Commission for providing City sewer services to property within unincorporated territory located at 4136 Mountain Drive. 2006-49 Resolution awarding a contract to Bonadiman-McCain, Inc. for street improvements per Revision No. One of Drawing No. 11393 at Chestnut Avenue from Station 14+21.53 to Irvington Avenue (SS -C 05/06), per Plan No. 11393. 2006-50 Resolution authorizing execution of a Reimbursement Agreement with the City of Riverside for street improvements to Commercial Road and Hunts Lane. 2006-51 Resolution authorizing the execution of a Contract and issuance of an annual purchase order with two one-year options to A Security Max, Inc. for security guard services at the San Bernardino Metrolink Station in the amount of $119,450. 2006-52 Resolution authorizing the execution of a purchase order to Fairview Ford for the purchase of two 2006 F-250 4WD crew cab pick up trucks, and one 2006 F- 250 2WD crew cab pick up truck to be utilized by the Public Services Department. 2006-53 Resolution awarding big to Matich Corporation for paving services, labor and materials, and increase a purchase order to Matich Corporation an additional amount of $2,781,700.00 for a not to exceed total amount of $4,263,000.00 2006-54 Resolution authorizing an increase to purchase order for Pavement Recycling Systems Inc. 2006-55 Resolution authorizing the execution of a purchase order to Turf Star, Inc. for a Toro riding mower to be utilized by the Parks and Recreation Department. 2006-56 Resolution authorizing the execution of the Fifth Amendment to the Limited Access Easement Agreement between the City and Rancon Realty Fund IV to extend the term of Agreement relating to the former Waterman Landfill. 2006-57 Resolution re -adopting an Enforcement Response Plan establishing appropriate responses and penalties to non-compliant users who discharge non- domestic wastewater to the San Bernardino Water Reclamation Plan and repealing Resolution No. 2006-35. 6 Tuesday, February 21, 2006 (cont.) 2006-58 Resolution authorizing the Mayor to execute an Agreement with GFR Enterprises, Inc. for the dedication and improvement of a 4 -acre park located on the southeast corner of Irvington Avenue and Chestnut Avenue. (REPEALED BY RES. 2010-72) 2006-59 Resolution authorizing the Mayor to execute a Parks Dedication and Improvement Agreement with J.R. Watson and Associates Development Co. for six parks and trail systems. 2006-60 Resolution authorizing the Police Department to apply for an Indian Gaming Local Community Benefit Grant in the amount of $936,000, and authorizing the expenditure of said grant funds for designated law enforcement purposes. 2006-61 Resolution proposing an amendment to the Charter of said City of San Bernardino relating to the salary of Council Members. 2006-62 Resolution calling a Special Municipal Election on a proposed Charter Amendment relating to the salary of the Council Members and requesting that said City of San Bernardino's Special Municipal Election be consolidated with the June 6, 2006 Statewide General Election conducted by the County of San Bernardino. 2006-63 Resolution submitting to the qualified electors of said City for their approval a proposed Charter Amendment relating to the salary of Council Members; transmission of the proposed Charter Amendment to the Office of the city Attorney for purposes of preparation of an impartial analysis. 2006-64 Resolution adopting General Plan Amendment No. 05-12 to the General Plan of the City of San Bernardino, to amend the General Plan Land Use Designation from CO -1, Commercial Office to CG -2, Commercial General, for one parcel located on the north side of 3rd Street, approximately 240 feet east of Sierra Way. Monday, April 3, 2006 2006-87 Resolution approving the destruction of certain records no longer required to be maintained by the San Bernardino City Attorney's Office. 2006-88 Resolution authorizing the execution and delivery of the Facility Lease and the Site Lease by and between the City and the California Infrastructure and Economic Development Bank and approving certain other matters in connection therewith. 2006-89 Resolution authorizing the execution of an Agreement with Communications Support Group, Inc. (CSG) to conduct a review of franchise fees and utility user taxes (UUT) paid by Charter Communications. 2006-90 Resolution amending Resolution No. 3985 entitled, in part, "a Resolution... prohibiting parking upon certain designated streets, alleys, or portions thereof..." to establish no parking any time zones on the west side of Arrowhead Avenue between Fifth Street and one -hundred eighty (180) feet south of Fifth Street and on the south side of Fifth Street between "D" Street and Arrowhead Avenue. 2006-91 Resolution preliminarily determining that the public convenience and necessity require the formation require the formation of a landscape maintenance and street lighting assessment district located in the San Bernardino International Airport/Alliance-California area, generally north of Central Avenue, east of Lena Road and south of 3'd Street, without compliance with the Special Assessment Investigation, Limitation and Majority Protest Act of 1931; declaring its intention to proceed to order with within the district, to be known as Assessment District No. 1022, Zone 1, Zone 1, and Zone 3; preliminarily approving the Engineer's Report and giving notice of a public meeting and public hearing. 2006-92 Resolution authorizing the San Bernardino City Fire Department to apply for and administer the FY 2006 Assistance to Firefighters Grant Program submitted to the Department of Homeland Security in the amount of $601,266. 2006-93 Resolution authorizing the Mayor or his designee to execute Amendment No. 1 Contract 05-689 with the County of San Bernardino for a 1 percent COLA increase to the FY 2005-2006 budget for the Head Start/Preschool Program. 10 Monday, April 3, 2006 (cont.) 2006-94 Resolution authorizing the purchase of 13 "Pocket Citation" electronic citation writing devices from Advanced Public Safety (APS). 2006-95 Resolution authorizing the execution of a Memorandum of Understanding with the Department of Justice California Armed Prohibitive Program Inland Empire Task Force. 2006-96 Resolution authorizing an increase to a purchase order to County of San Bernardino Solid Waste for street sweeper dumping. 2006-97 Resolution setting the amount of certain development impact fees, and repealing Resolution 88-140.
