Recommended motion:
That Change Order No. 1 to the contract with Doug Martin
Contracting Company, Inc. for the pavement rehabilitation by slurry
seal method, in accordance with Plan No. 9715 , be approved; to
provide for the removal and replacement of additional areas of
pavement asphalt failures , at an estimated increase in the contract
price of $8 , 590 . 00 , from $342 , 147 . 20 to $350 , 737 .-20 .

STAFF REPORT
Bid item No. 4 . , in the contract documents for pavement rehabili-
tation on various streets by application of slurry seal , is for
the repair of 8 ,000 square feet of failed areas. These failed
areas are normally old street cuts or potholes , and were to be
repaired by removing the asphalt concrete pavement and up to 2
feet of the subgrade.
After construction was commenced, the Contractor determined that
many of the distressed areas were merely a result of the pavement
being in a deteriorated condition, and that the subgrade in most
of these areas was still in good condition. However, the
Contractor has estimated that 18 , 200 square feet of pavement is
distressed, and needs to be removed and replaced.
The Contractor has submitted a proposal to saw cut and remove an
estimated quantity of 18 ,200 square feet of pavement, and replace
with new asphalt concrete, at a unit price of $2 . 45 per square
feet. This price will also include any necessary removal and
replacement of subgrade materials.
We have reviewed this proposal and found that it will result in a
better final product. Also, that the price quoted is fair and
reasonable for the work involved.
Change Order No. One will delete Item No. 4 , Street Cut/Pothole
Repair, for a credit of $36 , 000 . 00 , and authorize the removal and
replacement of an estimated 18 , 200 square feet of pavement, at a
cost of $44 ,590 . 00 . The differential of $8 , 590 . 00 will increase
the contract price from $342 , 147 . 20 to $350 , 737 . 20 .
The total estimated project cost, based upon the actual low bid
price, contained an amount of $17 , 852 . 80 for contingencies, which
can be used to finance the additional cost of this change order.
All costs incurred due to this change order will be charged to
the $500 , 000 . 00 allocated under Account No. 126--369-5504--1520 .
We recommend that Contract Change Order No. One be approved.

Gentlemen:
You are hereby authorized and directed to make the following changes
to the contract to incorporate the change of work, as follows:
(1) Delete Contract Bid Item No. 4, Street Cut/Pothole Repair, at
the contract bid price of $ 4.50, for 8, 000 S.F. , for a total
credit of < $ 36, 000. 00 >.
(2) Removal and replacement of Asphalt Concrete Pavement without
the installation of Aggregate Base and recompaction of the
subgrade where the subgrade is firm and sound, and the
re-compaction of the subgrade where it is found to be unstable,
for a re-negotiated price of $ 2 .45 per S.F. for 18, 200 S.F. ,
at a total cost of $ 44 , 590.00.
