AMENDING SAN BERNARDINO MUNICIPAL CODE SECTION
10.24.190(A) TO PROHIBIT ALL COMMERCIAL VEHICLES EXCEPT FOR
COMMERCIAL PICKUP TRUCKS, VANS, AND PASSENGER CARS ON
MT. VERNON AVENUE BETWEEN 5TH STREET AND 2ND STREET (MT.
VERNON AVENUE BRIDGE).
The existing Mt. Vernon Avenue Bridge (sometimes referred to as "Mt. Vernon Avenue
Viaduct") was constructed by the State in 1934. Some of the steel framing for the bridge was
salvaged from the previous bridge, which was built by the railroad in the early 1900's.
In 2004, Inspectors from the Caltrans Office of Structure Maintenance & Investigations
discovered cracks in the steel girders at several locations under both the north and southbound lanes
of traffic. Based upon the recommendation ofCaltrans, the City closed the bridge temporarily until
it could be shored. In June of2004, shoring with heavy timbers was completed and the bridge was
re-opened with the restriction that 18 wheel trucks were prohibited from using the bridge.
In April of2008, Caltrans advised City staff that additional cracks had been discovered in
several girders that were not shored. On August 26, 2008, the City received another inspection
report from Caltrans (copy attached) which identified newly discovered buckling in the center 3
girders of span 6. The City is in the process of designing and will install additional shoring in the
area of the new cracks and buckling.
On August 28, 2008, Robert Eisenbeisz, City Engineer, issued a memorandum (copy
attached) stating that based on information contained in the most recent Caltrans inspection report,
Mr. Eisenbeisz recommended that all commercial vehicles, except for commercial pickup trucks,
vans, and passenger cars be prohibited on the Mt. Vernon Bridge.
Recommended Motion: Adopt the Urgency Ordinance.

We recently received correspondence from Caltrans, dated July 15,2008, notifying the City that
they have performed routine inspections for 30 bridges under City jurisdiction. The bridges were
rated for deficiencies, structural adequacy, safe load carrying capacity and overall general
condition. Enclosed with this correspondence were inspection reports for each individual bridge
structure that was inspected.
Based upon the information contained.. in the Caltrans inspection report for the Mt. Vernon
Bridge, I recommend that large trucks be prohibited on Mt. Vernon Avenue between Sib Street
and 2nd Street, that the Municipal Code be amended to include this roadway segment in Section
10.24.190 (A); prohibiting commercial vehicles, except for commercial pickup trucks, vans, and
passenger cars, and that appropriate signs be erected as soon as possible giving notice of this
prohibition.
I further recommend that this prohibition remain in place until the new bridge is constructed.
These measures need to be implemented immediately to help prevent additional deterioration to
the existing bridge structure.

In accordance with Title 23 of the Code of Federal Regulations (Federal Highway Act) and the
National Bridge Inspection Standards (NBIS), Caltrans Structure Maintenance and Investigations
performed a Routine inspection for 30 bridge(s) under your jurisdiction. The bridges have been rated
to their deficiencies, structural adequacy, safe load carrying capacity and overall general condition.
Enclosed are copies of the Bridge Inspection Reports for the structures noted on the attached
transmittal sheet. These reports contain descriptions of physical changes to the structures since the last
inspection, recommendations for work to be done, or additional information not recorded in the
previous Bridge Reports.
Your attention is directed to the requirements of Title 23, Part 650 of the Code of Federal Regulations,
where newly completed structures or any modification of existing structures shall be entered in the
inventory within 90 days. Please notify this office of any newly constructed bridge or culvert within
your jurisdiction, more than 20 feet measured along the center of the roadway and carrying public
vehicular traffic or over a public roadway, in order that it may be entered in the inventory of bridge
structures in compliance with Federal requirements.

Inspection Type Definitions
Routine Inspection:
Routine Inspections consist of both the initial Inventory Inspection (the first inspection of the bridge
that places it in the bridge inventory or when there has been a change in the configuration of the
structure) and subsequent regularly scheduled inspections. The initial inspection provides all the
Structural Inventory & Appraisal (SI&A) data required by federal and state regulations, determines the
baseline structural conditions, lists any existing problems, and establishes the load capacity of the
structure. Subsequent inspections consist of observations, measurements needed to determine the
physical and functional condition of the bridge, to identity any changes from the previously recorded
conditions, and verification of its load capacity. These inspections are generally conducted from the
deck, ground and/or water level, and from permanent work platforms and walkways, if present.
Inspection of underwater portions of the substructure is limited to observations during low-flow
periods and/or probing for signs of undermining. Special equipment should be utilized in circumstances
where its use provides the only practical access to areas of the structure.
Fracture Critical, Special Feature & Underwater Inspections:
Fracture Critical, Special Feature, and Underwater Inspections are up close, hands-on inspections of
one or more members above or below the water level to identify any deficiencies not readily detectable
using Routine Inspection procedures. These inspections generally require special equipment such as
under-bridge inspection equipment, manlifts, boats, traffic control, and railroad flagging. Personnel
with special skills such as divers or structural steel inspectors trained in non-destructive testing
techniq ues may be required.
Other Inspections:
Other Inspections are conducted on damaged structures, structures that have developed specific
problems, or structures suspected of developing problems. The scope of these investigations should be
sufficient to determine the need for emergency load restrictions or closure of the structure, monitor a
changing condition, and to assess the level of effort necessary to effect a repair.

slab on closed end backfilled RC cantilever abutment on framed RC

The fracture critical inspection performed on 8/13/2007 identified 2 new cracks at bent 7
on the span 6 side in girders 2 and 4 (span 7 has temp shoring under at both bent 7 and
8). It was recommended via telephone conversations with the City of San Bernardino that
the situation be evaluated by the engineer of record and additional temp shoring be
installed on the span 6 side of bent 7.
During the inspection on 6/16/2008 it was discovered that the center 3 girders of span 6

are experiencing buckling at the skewed bent 7. Girder 4 appears to have. the most
deformation of the web section with girders 3 and 2 each having less than the previous.
The replacement brdige project is still in EIR phase with replacement to commence by late
2010 or 2011.

Since ntemporary'l shoring has been in place for approximately 3 years and it will likely
be in place until structure is replaced, the bottom beams that are sitting directly on
the ground (AC paving) should have their bottom sides inspected for possible rot/decay
from trapped moisture, etc. These beams should be rotated or replaced and placed on
concrete pads that will allow drainage and prevent rain run off from affecting timbers.
The temporary shoring on the south side of the bridge is all sitting directly on soil
under structure.
Most vertical members of the temporary shoring exhibit full length splits that run to the
center pulp of the elements, which appear to be due to wood curing/drying. The t~mporary
timber shoring is placed directly on the asphalt of the railroad yard and the area must
be kept clean and free of deleterious material that can lead to deterioration of the
timber members. Due to vibrations caused by traffic, the shoring wedges must be checked
regularly and tightened as needed to maintain bearing.
The temporary shoring system is beyond its design life and needs to he inspected and
recertified by the designer of record. This IItemporaryn system should be recertified on
an annual basis at a minimum. A signed and stamped letter certifying that the shoring
system is in proper condition and performs as designed should be on file with the city
engineerls office and with the BIR at Caltrans.

CONDITION OF STRUCTURE
Based on the report dated 4/29/2004 and field inspection on 5/8/2006, it appears that
this structure has undergone structure failure.
This existing structure failure locations are currently shored by timber members and the
shoring has been designed to take the full structure dead load and live load.
Timber wedges and tie rods were found to be loose on 8/13/2007 and subsequently reset and
tightened by Skanska personnel on 8/28/2007.
The condition of this structure is unchanged since the previous inspection except as
noted:
_ The steel finger joint of Bent 12 has vertical offset in a wave shape due to traffic
(60 mm max). (The steel finger joint is lost its functionality)
- The pourable j oint seal of Bents 8 and 12 exhibit "D'I crackings.
The deck of the Bent 19 exhibits 4 different uneven elevations between transverse and
longitudinal joints.
The east exterior steel girder of midspan 20 exhibits 3 m long impact damage from a
fork lift in rail yard. (The bottom flange and web are bent in the maximum moment area)
Most vertical members of the temporary timber shoring exhibit severe splits.
- Debris, soil and bird droppings are accumulating around bases of temporary bents.
- Several sections of both railings have been damaged and have temporary patches in
place.

PAINT CONDITION
On almost all painted steel elements it is difficult to determine the state of the paint
system. There are several areas that are so dark with diesel soot from trucks and trains
that it is impossible to tell if the paint system is still in place (bridge top coat is
black). There are also extensive effloresence stains on the steel, particularly at bent
caps, as a result of dripping down stringers and bent caps. It is obvious that there is
corrosion and deterioration in the many areas. The current condition states for all
painted elements is reasonable. (City of San Bernardino is in EIR phase for a replacement
bridge with anticipated construction bids in 2010.)
SAFE LOAD CAPACITY
Based upon the structural failures found during the "fracture critical" steel inspection
on April 22nd, 2004 we recommended on April 23rd, 2004 that the City close the two
southbound lanes until adequate repairs are made to the damaged bridge elements. The
City followed this recommendation immediately. We continued our detailed investigation of
the structure during the week of April 26th, 2004. The results of all these
investigations are presented in the "Group A Report" dated April 28, 2004 as well as in
the section "Condition of Structure" of this Report.
Based upon the structural failures mentioned in this report, the bridge was evaluated for
legal loads at the critical locations of deterioration that are currently open to
traffic. These include span #7 damage to girders #7 and #8 and bent cap #18.
It was noted in the report that stringers #7 and #8 of span #7 had 60 rom long web cracks
at the supports. These cracks are quite large relative to the depth of the stringer (350
rom) and even larger relative to the reduced depth of the stringer web at the connection
(approximately 250 mm). These cracks could continue to grow leading to failure of these
stringers. Based upon this assumption we evaluated this span assuming that these damaged
stringers were missing. When these girders are missing, the positive moment demand will
increase significantly and the deck slab would be inadequate to function due to the
insufficient quantity of bottom reinforcement. Consequently, this span would have no
live load capacity.
Bent #18 was also evaluated considering section loss at various locations of the top
flange. The analysis, which assumed a top flange thickness of 12.5 mm, indicates that the
bent cap is adequate to carry legal loads.
SIGNS
Silhouette-type signs, indicating the weight limits noted under IEXISTING POSTING', are
in place at each approach to the bridge.

We recommended that the City completely close the bridge until adequate repairs are made
to the damaged bridge elements. We communicated by phone the results of our analysis and
the closure recommendation to Mr. Mike Grubbs, City Engineer, on June 2nd 2004.

the Routine Inspection Report of 4/29/2004 and 5/8/2006.
HISTORY
During Special wGroup A" inspection (looking for potential cracks in steel elementsl on
April 22 2004, CALTRANS found major structural failures (consisting in severe cracks and
web bucklings) in several steel girders of span 20 as well as their connection to their
adjacent steel bent caps. Due to this deteriorated condition, on April 23rd, 2004,
Caltrans recommended that the City should close the two southbound lanes until adequate
repairs are made to the damaged bridge elements. The City followed this recommendation
immediately. Caltrans continued the detailed investigation of the structure during the
week of April 26th, 2004. These investigations revealed other major failureain the
girders of span 19, 18, 14, and especially in span 7.
Immediately, CALTRANS conducted a detailed engineering analysis to determine the load
capacity of the bridge with the account of the existing damaged state. It was determined
that keeping this bridge in operation even under restricted load will be a serious safety
hazard to traveling public on the bridge and activities of BNSF Railroad underneath.
All these findings and the closure recommendation were presented in two Bridge Inspection
Reports submitted to the City in May and June 2004. On June 3, 2004, following the
recommendation of CALTRANS, the City temporarily closed all traffic lanes on north and
south bounds of Mt. Vernon Bridge.
Section 35754 of the California Vehicle Code allows the City to keep a bridge closed for
a period of less than 90 days with temporarily erected suitable signs at all entrances to
the bridge, without public hearing.
The City had proposed to shore up the cracked girders so that the bridge will be capable
of carrying the legal live loads and to subsequently re-open the bridge to vehicular
traffic. However, due to unforeseen difficulties, the shoring work could not be completed
within the 90-day period allowed by Section 35754, which expired on 09/02/04.
Section 35751 of the California Vehicle Code extends the authority to keep a bridge
closed for a period in excess of 90 days, subject to specific notification, public
hearing and posting of signs as required by the Vehicle Code. Since the City of San
Bernardino has a population less than 1,100,000, the Vehicle Code requires that the
public hearing will be conducted by CALTRANS.
On 8/30/2004, Public Hearings were held on the engineering investigation of this
structure looking toward its closure until work is performed on the structure that will
make it capable of carrying the legal live loads. pursuing to these Hearings, an WOrder
for the closure of Bridge No. 54C0066 - Mt Vernon Overhead in the City of San Bernardinow
was issued by CALTRANS.
On 10/04/2004 the shoring work was completed and the bridge was re-opened to vehicular
traffic.

2 URGENCY ORDINANCE OF THE CITY OF SAN BERNARDINO AMENDING SAN
BERNARDINO MUNICIPAL CODE SECTION 10.24.I90(A) TO PROHIBIT ALL
3 COMMERCIAL VEHICLES EXCEPT FOR COMMERCIAL PICKUP TRUCKS, VANS,
AND PASSENGER CARS ON MT. VERNON AVENUE BETWEEN 5TH STREET AND 2~D

5 WHEREAS, the existing Mt. Vernon Avenue Bridge was constructed by the State in 1934,
with some of the steel framing salvaged from the previous bridge, which was built by the railroad in
6 the early 1900's; and
7 WHEREAS, in 2004, Inspectors from the Caltrans Office of Structure Maintenance &
Investigations discovered cracks in the steel girders at several locations under both the north and
8 southbound lanes of traffic. Based upon the recommendation ofCaltrans, the City closed the bridge
temporarily until it could be shored. In June of2004, shoring with heavy timbers was completed and
9 the bridge was re-opened with the restriction that 18 wheel trucks were prohibited from using the
bridge; and

WHEREAS, in April of 2008, Caltrans advised City staff that additional cracks had been
II discovered in several girders that were not shored;
12 WHEREAS, on August 26, 2008, the City received another inspection report from Caltrans
which identified newly discovered buckling in the center 3 girders of span 6. The City is in the
13 process of designing and will install additional shoring in the area of the new cracks and buckling; and
14 WHEREAS, on August 28, 2008, Robert Eisenbeisz, City Engineer, issued a memorandum
stating that based on information contained in the most recent Caltrans inspection report, Mr.
15 Eisenbeisz recommended that all commercial vehicles except for commercial pickup trucks, vans and
passenger cars be prohibited on the Mt. Vernon Avenue Bridge;

NOW THEREFORE, the Mayor and Common Council of the City of San Bernardino do
17 Ordain as Follows:
18 Section I. San Bernardino Municipal Code Section 1 0.24.190(A) is hereby amended to
add Subsection (23) to read as follows:

Pursuant to the authority and restrictions of Vehicle Code Sections 35701, et
seq., no person shall operate or drive any commercial vehicle upon the

Section 2. . This urgency ?rdinance shall take effect upon adoption and is adopted pursuant
to San Bemardmo City Charter Section 31 and 121, by a two-thirds (2/3) vote of the Council forthe
immediate preservation of the public peace, health, or safety. The Mayor and Common Cou~cil find
that all of the above-stated recitals are true and hereby incorporate them herein as the statement of
urgency supporting this ordinance.
