Subject:
Authorize the Director of Public Works to execute Contract Change Order No. Two with
Hillcrest Contracting, Inc. for the Victoria A venue Street Improvement and Beautification
Project Phase II, per Plan No. 12321 (SS05-25).

On July 6,2010, the Mayor and Common Council adopted Resolution No. 2010-231, awarding a
contract to Hillcrest Contracting, Inc. for construction of street widening improvements on
Victoria Avenue between Highland Avenue and 28th Street. The Notice to Proceed was issued to
the contractor on September 13, 2010, and work began shortly thereafter.
During the grading stages for widening Victoria Avenue near the Patton Hospital entry, the
contractor discovered a large amount of asbestos which required disposal as a hazardous
material. The removal was done by Brickley Environmental, a local environmental engineering
firm that specializes in hazardous wastes. The contractor was instructed to remove the material
before continuing with grading operations. In order to facilitate its removal, additional traffic
control became necessary to accommodate the truck movements, and production time was lost
during the emergency removal. This change order provides for these unforeseen costs.
After removal of the existing curb and gutter on the south side of Highland A venue in
preparation for widening the street to provide an additional dedicated left-turn lane at Victoria
Avenue, staff worked with the contractor to modify the design to provide driveway access at
26473 Highland Avenue. The reduced setback for this structure due to the relocated curb would
not allow construction of a drive approach. This change order provides for the contractor's
additional work efforts to re-construct the driveway and approach.
During construction of the parking lot improvements, a gas line was discovered under an existing
concrete stoop at the rear of 26461 Highland Avenue. Changing the building's rear entry
required additional work and an increase in quantities from the original bid proposal. This
change avoided a costly time delay due to the often slow reaction times for Southern California
Gas crews in making modifications like these. This Change Order provides for the contractor's
increased work and quantities to provide this modified entry.
After completion of the paving on the South side of Highland A venue, it was determined that
additional asphalt grinding and paving was required to encompass all of the distressed pavement
that existed on both Highland A venue near Victoria A venue and on Victoria A venue from
approximately the entry to Patton Hospital south to Highland Avenue. This work was not
included in the original contract plans and specifications.
