ADDENDUM #1
TO
l1lequest for Quotation (RFQ) F-10-08
, Mobile Command Vehicle
Notice is hereby given that the following changes, additions, deletions, or clarifications
are incorporated as an A~dendum to the Request for Quotation (RFQ) F..10-08
1. Please clarify if the bond is a Bid or Performance bond. Also, will this bond be
required after awaRt of contrad? If so, it can take up to two-three weeks to file
the paperwork an~ secure the bond. As part of the bond, will you require" a
specific completio~ date?
ANSWER:'" performance bond is required by the U.S. Department of
Homeland ~rity (DHS) for any equipment over $250,000, or any
vehicle pu~ased with DHS grant funds. The performance bond is to
guarantee tie satisfactory completion of the projed. The cost of the
performan~ bond may be built into the cost of the equipment, and is an
allowable grant expense.
The performance period to complete this project is March 31,2010. An
extension may be granted for compelling reasons.
2. Is there any possi~ility of the mandatory 100% Faithful Performance Bond being
eliminated or even reduced for this projed? We understand that it's in the best
interest of the city ito have a vendor who can produce the best vehicle possible,
but we feel this r~uirement, at least in full, will inhibit a large majority of potential
vendors to bid on the project, especially small companies like ours.
2009-353
ANSWER: 'A performance bond is required by the U.S. Deparbnent of
Homeland ~rity (DHS) for any equipment over $250,000, or any
vehicle pu,*hased with DHS grant funds. The performance bond is to
guarantee "e satisfactory completion of the project. The cost of the
performan~ bond may be built into the cost of the equipment, and is an
allowable grant expense.
3. You stated that y~r grant deadline is March 31,2010. Will it be possible to ask
for an extension to your grant? Our standard lead time is 180 days after receipt
of purchase order ~ but due to the radio and interoperability equipment it could
take up to 210 days.
ANSWER: lYes, we may request an extension through the California
Emergency! Management Agency for compelling reasons.
4. Our standard te~ are: 50% DOWN TO START AND ORDER CHASSIS. THE
ADDITIONAL 50~ BALANCE WILL BE DUE UPON COMPLETION AND
ACCEPTANCE AT USV FACTORY AND PRIOR TO DELIVERY. We can extend
the 50% balance ~ue to Net 30 days. Is this acceptable for the City?
ANSWER: This is a requirement for the grant.
5. Are we required tQ obtain a City of San Bernardino Business License if awarded
the contract? If so~ what is the cost of the Business License?
ANSWER: rfes, cost varies - please contact the City Clerk's office at
(909) 384-$302 for further information.
6. We would like to s~bmit an alternative bid, using our standard construction. We
do not build RVs ,nd it appears the basic platform for this project is similar to
that construction. pur construction is designed to be worked in, and last for many
years of hard use,! typical in the emergency service industry. Some of our
vehicles under dai~y hard use, have been remounted more than once, providing
excellent long te~ value to our customers and their community.
ANSWER: ~es, we would accept your bid for consideration.
7. In reference to RFP F-10-08, p.27 "12 Channel Voice..." will you considered
"Equal I Better" pr~ucts other than manufactured by Telex?
ANSWER: Yes, simplicity of operation is however a basic principle and
requireme~. This equipment will be shared with other deparbnents within
the Rivers~e Urban Area Security Initiate zone; the primary cities being
Riverside, ~an Bernardino and Ontario.
2
2009-353
8. We were browsin~ through the technical specifications and saw that you are
interested in a 09/~ 0 36' Class A with the Workhorse chassis. The only problem
we have is that Wi~nebago did not make a 36' Class A with the Workhorse
Chassis in 09/10. iThis is no problem if you jump down to a 33' or up to a 38'.
Could you please respond and let me know which two options would better suit
your needs? I woUld imagine the 38' would be the better fat, but again, just trying
to get your input. (4.ny information would help!
ANSWER: We are working within the standards of a Type II mobile
command .hicle with a chassis requirement of 35'-40'; therefore your 38'
chassis woljJld be appropriate.
Sincerely,
Deborah R. Morrow, C.P JM., MPA

CHAPARRAL Class A's before closing the company and starting Universal
Specialty Vehicles, Inc. in 1994. Andy and Mary decided to start
specializing in custom-built Mobile Units instead of trying to compete in the
heavily saturated Class A R V market.
Andy and Mary Hall have assembled a management and production team,

manufacturing Mobile Command/Communications Vehicles and Trailers.

Riverside and the Escondido Police Department contracts required
performance bonds. The projects were completed on time and within the
time frame established by the bond requirements.
At Universal Specialty Vehicles, Inc. we work closely with our customers to
provide them with a product that meets their specific needs. We have
worked with many agencies using grant funds and understand the
requirements set forth by the grant agencies.

FonJIgn person. If you are a foreign person, do not use Form
W-9. Instead, use the appropriate Form W-8 (see Publication
515, Withholding of Tax on Nonresident Aliens and Foreign
Entities).
Nor........ alien who becomes a r8Sident alien. Generally,
only a nonresident alien individuaJ may use the terms of a tax
treaty to reduce or eliminate U.S. tax on certain types of income.
However, most tax treaties contain a provision known as a
-saving clause.8 Exceptions specified in the saving clause may
permit an exemption from tax to continue for certain types of
income even after the payee has otherwise become a U.S.
resident alien for tax purposes.
If you are a U.S. resident alien who is retying on an exception
contained in the saving clause of a tax treaty to claim an
exemption from U.S. tax on certain types of income, you must
attach a statement to Form W-9 that specifies the following five
items:
1. The treaty country. Generally, this must be the same treaty
under which you claimed exemption from tax as a nonresident
alien.
2. The treaty article addressing the income.
3. The article number (or location) in the tax treaty that
contains the saving clause and its exceptions.
4. The type and amount of income that qualifies for the
exemption from tax.
5. Sufficient facts to justify the exemption from tax under the
terms of the treaty articte.
&am,af& Article 20 of the U.S.-china income tax treaty allows
an exemption from tax for schoIa'ship income received by a
Chinese student temporarily present in the United States. Under
U.S. law, this student wiD become a resident alien for tax
purposes if his or her slay in the United States exceeds 5
calendar years. However, paragraph 2 of the first Protocol to the
U.S.-china treaty (dated April 30, 1984) allows the provisions of
Article 20 to continue to apply even after the Chinese student
becomes a resident alien of the United States. A Chinese
student who qualifies for this exception (t.nder paragraph 2 of
the first protocol) and is relying on this exception to claim an
exemption from tax on his or .. scholarship or fellowship
income would attach to Form W-9 a statement that includes the
information desaibed above to support that exemption.
