Description: Prop 1 B Finance: Council Notes: Agenda Item No. r�-lm-oR CITY OF SAN BERNARDINO - REQUEST FOR COUNCIL ACTION STAFF REPORT Subject: Authorization for the Director of Development Services to execute Contract Change Order No. Two with Matich Corporation for the Realignment of Viaduct Boulevard between 2nd Street and 3`d Street, per Project Plan Nos. 12325 & 12326. Background: On February 17, 2009, the Mayor and Common Council approved a Resolution awarding a contract to Matich Corporation for the Realignment of Viaduct Boulevard between 2"d Street and 3rd Street. This project consists of removal of the existing Viaduct Boulevard between 2nd Street and 3`d Street and reconstruction of the realigned road to create a 90-degree intersection opposite Giovanola Street at 2nd Street. This realignment is necessary to accommodate the new Metrolink Parking Structure that will be starting construction in early 2010. A traffic signal has been installed at the intersection of Viaduct Boulevard and 2nd Street as part of the project. On July 6, 2009, the Mayor and Common Council authorized the Director of Development Services to execute Contract Change Order No. One with Matich Corporation for the Realignment of Viaduct Boulevard between 2nd Street and 3rd Street. Construction Change Order No. Two: During construction the contractor was directed to complete additional work and/or encountered unforeseen conditions, such as discovery of abandoned railroad ties buried in the existing roadway, discovery of unsuitable material in 2nd Street that had to be removed and replaced, removal and replacement of damaged curb and gutter, relocation of an undocumented conflicting utility, and other tasks that were not included in the contractor's bid. The descriptions and justification of these items are detailed in the attached copy of Contract Change Order No. Two. Financial Impact: The revised estimated project costs are as follows: TOTAL Original Contract Amount $ 1,135,311.00 Adjustment due to reduced/deleted items $33,901.03 Contract Change Order No. One $ 21,625.00 Contract Change Order No. Two $ 164,016.54 Total Construction Contract Amount $ 1,287,051.51 This change order represents 14.4% of the original contract amount. The total of all change orders to date equals 16.4% of the original contract amount. Relocation of Streetlight Pole 2-2 A streetlight pole located at the southwest corner of Metrolink Way and 3rd Street was identified as a Phase 11 item, but it needed to be relocated as part of Phase I to avoid conflict with the new improvements. Relocation of Streetlight Conduit 2-3 During construction,an existing streetlight conduit was discovered to be in conflict with the installation of an under sidewalk parkway drain. The contractor was directed to relocate the streetlight conduit to cross under the new drain. Removal of Existine Palm Tree 2-4 The contractor was directed to remove an existing palm tree located on the northwest corner of Metrolink Way and 2nd Street because it was in conflict with the proposed improve ents and it was not included in the contractor's bid. Removal of Railroad Ties 2-5 During the grading operation, the contractor encountered buried railroad ties in the roadbed of 2nd Street and Viaduct Avenue. The contractor was directed to remove and dispose of the railroad ties. 2 CHANGE ORDER NO.TWO-MATICH CORPORATION REALIGNMENT OF VIADUCT BOULEVARD BETWEEN 2ND STREET AND 312D STREET,PER PROJECT PLAN NOS. 12325&12326. Adjustment of the South Side of Yd Street During construction, it was determined that the improvements along the south side of 3`d 2-6 Street needed to be elevated to match the new improvements installed by the abutting development (La Placita) and that the design needed to be adjusted to match the new existing conditions. The contractor was directed to re-grade the street to match the existing improvements. Removal&Replacement of Curb and Gutter Increase in Quantity of Bid Item No. 9, Schedule 1,6-inch P.C.C. Curb and Gutter 2-7 (272 L.F.@$26/L.F.) The contractor was directed to remove and replace existing curb and gutter that was in poor condition on the south side of 2nd Street. Removal and Replacement of Unsuitable Sub-Grade Material During reconstruction of 2 nd Street, the subgrade material was determined to be4 2-8 unsuitable and could not be properly compacted. The contractor was directed to remove the unsuitable material and replace it with Geo-grid fabric and Class II Base material to stabilize the subgrade. Removal&Replacement of Existine Metrolink ParkinE Area During construction it was determined that a portion of the existing Metrolink parking 2-9 area needed to be removed and replaced in order to provide proper drainage and avoid undesirable steep slopes in transitioning to the new improvements. The new grades required adjustment of an existing monitoring well to be flush with the pavement surface. Additional traffic striping was also necessary as a result of this change. Construction of Retainine Curb 2-10 The improvements along 3` Street resulted in an excessive elevation difference between 3`d Street and the existing parking lot. A retaining curb was needed to make up the difference in elevation. Storm Drain Conflict During construction of the storm drain, the contractor discovered that the elevation of the existing pipe flow line did not match the elevation shown in the plans, based upon 2-11 "as-built" record drawings on file. The contractor was required to remove approximately 40 feet of storm drain and a manhole, in order to adjust the slope of the new pipe to match the existing pipe. The contractor also was required to protect against flooding during construction of the storm drain. The protection included a barrier to prevent flooding. Adjustment of Top of Electrical Vault 2-12 The south side of 3` Street was lowered from existing conditions and this required the contractor to lower the top of an existing utility vault to match the new grades. Increase in Quantity of Bid Item No. 21, Schedule 1, Class II A.B. (28 CY Q) 26/C 2-13 The contractor was directed to remove unsuitable materials in the subgrade of 2nd Street. It was replaced with Class II aggregate base material. The quantity was based on field measurement. Increase in Quantity of Bid Item No. 5, Schedule 2, 24-inch RCP (10 L.F. (a, 2-14 92/L.F. & Increase in Quantity of Bid Item No. 6, Schedule 2, 18-inch RCP (24 & L.F.(u) $74/L.F.) 2-15 A catch basin located at Metrolink Way and 3`d Street was relocated to allow the local depression to more uniformly blend with the curb and gutter, avoiding an abrupt change in alignment. This relocation required the laterals to be extended. Repair/Replace Existing Curb and Sidewalk 2-16 The contractor was directed to replace damaged or irregular curb, gutter and sidewalk on 2nd Street at the Connection with Mt. Vernon Avenue. 3 CHANGE ORDER NO.TWO-MATICH CORPORATION REALIGNMENT OF VIADUCT BOULEVARD BETWEEN 2ND STREET AND 3RD STREET,PER PROJECT PLAN NOS. 12325&12326. Repair of Landscaping,Removal of Trash&Installation of Lava Rock 2-17 The contractor was directed to repair landscaping on the south side of 3`d Street and remove trash from the existing rock-lined channel. Lava Rock was installed in the landscape area instead of PCC, resulting in cost savings. Extension of Traffic Sianal Interconnect 2-18 The contractor was directed to install the traffic signal interconnect conduit under Giovanola Avenue. Protection of Water Services 2-19 The contractor was directed to install barricades and other devices to protect the water services needed for Phase Il. Increase in Ouantity of Bid Item No. 22, Schedule 1, Adjust manhole to Grade (1 2-20 ea. (i�$1,100) The contractor was required to adjust the top of one additional manhole to be flush with the pavement surface. Increase in Ouantity of Bid Item No. 19, Schedule 1, 8-inch A.C. Dike (154.5 L.F. (& $22/L.F.) The contractor installed additional A.C. dike instead of a concrete drain behind earth 2-21 stockpile to eliminate debris from leaving the site during a storm event. A temporary A.C. dike was needed to control draining on the existing Viaduct sheet between project phases. This item replaced the concrete V-gutter specified on the plans for a net project savings. Increase in Ouantity of Bid Item No. 16, Schedule 1, P.C.C. Cross Gutter and 2-22 Spandrel The contractor was required to install additional cross gutter and spandrel at 2nd Street and Metrolink Way, because field measurement was different from contractspecs. Additional Dust Control 2-23 The contractor was required to provide dust control measures at all times, including times when the contractor was unable to proceed with work, due to circumstances beyond the contractor's control. Additional Survey 2-24 The contractor was required to provide additional survey for the relocation of a catch basin southwest corner of 3`d Street and Metrolink Way see Items No. 2-14&2-18). 36-inch Sewer Manhole Frames & Covers 2-25 The contractor was required to supply additional materials needed to adjust existing manhole structures to be flush with the finish surface. Repair of Existing Parking Lot Lights The contractor relocated two(2)existing parking lot lights that were in conflict with the 2-26 new improvements. Upon completion of the work, the lights did not function properly and it was determined that the ballasts were faulty. The contractor replaced the ballasts and repaired the lights so that they function properly. It is unknown if the lights worked prior to the beginning of construction. JUSTIFICATION: ITEM NO: JUSTIFICATION UNKNOWN UTILITY TRENCH The contractor removed an existing unmarked and 2-1 undocumented utility in conflict with the water main. WATER BARRIER FOR STORM DRAIN CONFLICT The contractor installed a water barrier 2-2 to protect the storm drain system from probable rain. LIGHT POLE REPLACEMENT AND RELOCATION The contractor relocated a light pole and 2-3 adjusted a light standard to grade. This work moved from Phase II to Phase I. RELOCATE STREET LIGHT CONDUITS IN GRADE The contractor relocated shallow street 2-4 light conduits in grade. REMOVE EXISTING PALM TREE The contractor removed an existing palm tree from the NW 2-5 corner of 2nd Street and Metrolink Way. ADJUST MONITORING WELL TO GRADE The contractor adjusted a monitoring well to 2-6 grade in the Metrolink Parking area. REMOVE RAILROAD TIES The contractor removed undocumented buried railroad ties 2-7 discovered under Viaduct Road and 2nd Street. ADJUST ELEVATION SOUTH SIDE OF 3RD STREET The contractor adjusted the elevation 2-8 of the south side of 3rd Street to accommodate recently installed infrastructure. REMOVE AND REPLACE CURB AND GUTTER,SOUTH SIDE OF 2ND STREET The 2-9 contractor removed and replaced undulating curb and gutter on the south side of 2nd Street. REMOVE AND REPLACE UNSUITABLE SUBGRADE,2ND STREET The contractor 2-10 removed and mitigated unsuitable subgrade discovered in 2nd Street. REMOVE AND REPLACE METROLINK PARKING AREA The contractor removed and 2-11 replaced a portion of the Metrolink Parking area to meet current drainage design requirements and desirable grades. CONSTRUCT RETAINING CURB The contractor constructed a retaining curb north of 3rd 2-12 Street versus an enbankment. Stamped colored concrete was deleted from the contract offseting the cost by$7480. STORM DRAIN CONFLICT The contractor adjusted storm drain elevation due to a discrepancy 2-13 between researched available documents and actual invert elevation. ADJUST ELECTRICAL VAULT TO GRADE The contractor adjusted an electrical vault to 2-14 grade. REPAIR/ADJUST CURB AND SIDEWALK AT MT.VERNON BRIDGE The contractor 2-15 repaired and adjusted irregular curb and sidewalk at the Mt.Vernon Bridge. EANDSCAPE REPAIRS TMUTH SIDE OF 3RD ST.), TRASH REMOVXE—(IMCK DITCH),LAVA ROCK The contractor repaired landscaping on the south side of 3rd Street, 2-16 removed trash in the existing rock-lined drainage ditches to facilitate storm water flow,and purchased/installed additional lava rock in landscaped areas. Lava rock installed versus PCC resulted in a cost savings to the project. EXTEND INTERCONNECT TO COMPLETE CIRCUIT The contractor installed interconnect 2-17 conduit under Giovanola Avenue.
