designee to execute a grant agreement with the California Endowment in the amount of $148,860 to
provide youth mentoring services for the period of December 1,2000 through November 30,2002.

The Mentoring Program was established in April of 1995 as a project of the Mayor's Youth Task Force.
The initial funding came from the City and the San Bernardino City Unified School District as a
partnership to provide both mentoring and the Late Night Hoops Program. After this initial funding, the
Parks, Recreation and Community Services Department pursued grant funding to sustain the mentoring
program.
The department was successful with two grants. The department and the mentoring program became a
collaborative partner with San Bernardino County Public Health Department in developing and receiving
a Community Challenge Grant through the State of California Department of Health Services. This grant
initially supported mentoring from February 1997 through January 1999. A three-year renewal of the
grant was awarded in June 1999, covering the period of July 1999 through June 2002.
The Department also worked with the San Bernardino City Unified School District to apply for and
receive a grant from the Office of Juvenile Justice and Delinquency Prevention. This grant supported
mentoring from September 1997 through March 2000. The OJJDP grant expired on March 31, 2000.
The city was not eligible to reapply. The mentoring program was able to continue to operate, however,
due to a surplus carryover in OJJDP funds in the amount of $57,427. In the anticipation of the
discontinuance of the OJJDP funds, the department once again teamed with the school district to pursue
alternative funding. After much strategy, an application was developed for the California Endowment.
The application was submitted to the Endowment in February 2000. The department negotiated with the
Endowment from February through October, making major revision in program objectives, funding, and
other issues.
In October, the department was advised that the grant application would be awarded. The grant will fund
the Mentor Coordinator at 50 percent of salary and benefits (The County Health Department Community
Challenge Grant funds the remaining 50 percent). Additionally, two part-time Outreach Workers are fully
funded to include benefits. The Outreach Workers will visit mentees and their families and provide
assessments and referrals to increase access to primary care, mental health services, and substance
prevention and treatment. Funding also covers general operating costs, such as mileage, postage, and
supplies and contracted evaluation services.
Financial Impact
There will be no general fund cost to the City. The grant will provide $148,860 to cover program costs
from December 1,2000 through November 30,2002.

identification of risk factors for children and achieve a minimum of a 75% competency
level.

IV. EVAI.IJATlON:
The City of San Bernardino will subcontract with an evaluation consultant to work with the
project staff to develop the evaluation plan. The evaluation protocols will include both
proces$ and outcome evaluation measllTCS such as pre and post surveys for MentorslMentees
and parcntslguardian&, examination of supporting docwnentation (school records, case
management file, and assessments) and change from baseline in access to private and public
health resources. The evaluation plan wilJ be developed within the first 3 months of the grant
period and approved by TCE staff.

THE CALIPOaNIA ENDOWWBNT
STANDARD GRANT COMMITMENT TERMS
The California Endowment is a California nonprofit public benefit corporation,
and is a private foundation for purposes of the Intemal Revenue Code. This Standard
Grant Commitment Terms ("Commitment") refers to and incorporates conditions for
grants awarded by The California Endowment, to grantees of The California Endowment.
The California Endowment's mission is to expand access to affordable, quality
health care for underserved individuals and communities, and to promote fundamental
improvements in the health status of all Californians. The California Endowment seeks
to achieve its mission primarily through its grant making programs.
I. PVRPOSE OF A WARD.
The California Endowment has made a grant to the Grantee for the purposes set
forth in the Purpose of Grant Award (attached document).
II. AMOUNT AND TERM OF A WARD.
The amount of the award and the time for performance of activities are set forth in
the Purpose of Grant Award.
III. CONDITIONS OF AWARD.
Grantees of The California Endowment agree to accept the award only under the
following conditions:
A. Identification of The California Endowment. Grantees shall ensure that The
California Endowment is clearly identified as a support organization on all
published material relating to the subject matter of the award. All extemal
communications regarding The California Endowment shall be subrnitted first
to The California Endowment for its review and approval. (See Attached
Communications Guidelines).
B. Expenditure of Funds. Grantee shall expend the funds only for charitable
purposes, as described in the Purpose of Grant Award. Any funds not
expended in accordance with the terms of the grant must be returned to The
California Endowment within thirty (30) days following the grant's
termination date. Grantee may not use any of its grant dollars for purposes
other than those stated in the Purpose of Grant Award and in Internal Revenue
Code Section 4945(d)(I) through (5), inclusive, more particularly as follows:
I. To carry on propaganda or otherwise to attempt to influence legislation;
2. To influence the outcome of or participate in any public election or to
carry on, directly or indirectly, any voter registration drive;
3. To regrant an organization unless such organization is described in
C Internal Revenue Code Section 509(aXI) through (3), inclusive, or is an
exempt operating foundation as defined in Section 4940 (d)(2);
4. For purposes other than those which are religious, charitable, scientific,
literary, or educational within the meaning of Section 170(c)(2)(B) of the
Intemal Revenue Code; or
5. For purposes other than those stated in the Purpose of Grant Award.
C. Grantee's Status as an E1iiible Grantee.
1. This grant is specifically conditioned upon Grantee's status as an eligible
grantee of The California Endowment. Grantee warrants and represents
that it is a tax exempt organization under section 501 (c)(3) of the Internal
Revenue Code, and is not a private foundation as defined in section 509(a)
of the Internal Revenue Code or is an exempt operating foundation
described in Section 4940(d)(2) of the Internal Revenue Code. Grantee
has provided The California Endowment with a copy of an IRS
Determination Letter evidencing its status as an eligible grantee. Grantee
will notify The California Endowment immediately of any change in tax
status, including any substantial or material change in sources of support
for any taxable year affected by this Commitment, within the meaning of
C Section 1.170A-9 of the IRS Treasury Regulations. Grantee warrants and
represents that this Commitment shall not change Grantee's status under
Section 509(a) of the Internal Revenue Code.
2. The forgoing subparagraph C.l. above shall not apply if Grantee is a
political subdivision of the State of California, or is otherwise an
organization described in Section 170(c )(1) or Section 511(a)(2)(B) of the
Internal Revenue Code.
D. Return of Funds. Grantee shall return to The California Endowment any
unexpended funds granted under the following conditions:
1. If The California Endowment, in its sole discretion, determines the
Grantee has not performed in accordance with this Commitment and/or the
Grantee's grant program or budget as approved by The Califomia
Endowment; or
2. If grantee loses its status as an eligible grantee as described in Section C.l.
or C.2. above.
3. Any portion of the funding not used for grant purposes.
C E. Funds Records. Audit, Site Visits. Funds provided by The California
Endowment shall be accounted for separately in the Grantee's books and
records. A systematic accounting record shall be kept by the Grantee of the

receipt and disbursement of funds. The Grantee shall retain original
substantiating documents related to grant expenditures and make these records
available for The California Endowment's review upon request. Grantee shall
be responsible for maintaining adequate financial records of this grant
program. The California Endowment, or a designated representative, reserves
the right, upon written notice, to audit Grantee's books and records relating to
the expenditure of any funds provided by The Califomia Endowment.
F. Budllets. Expenditures of grant funds must be only for line items specified in
the approved grant budget. Advance written approval by The California
Endowment must be received by the Grantee for disbursements deviating
from the line items of the approved budget. Any interest earned on grant
funds must be applied to the project purposes and must be reported to The
California Endowment.
G. No Further Oblillations QY The California Endowment. This grant is made
with the understanding that The Califomia Endowment has no obligation to
provide other or additional support or grants to the Grantee.
H. Public RE:Portinll. Grantee agrees to disseminate to interested publics, by
using established channels of communication pertinent information relating to
the results, findings, or methods developed through this grant. (See Attached
Communications Guidelines).

I. Intellectual Prqperty. All copyrights and all other intellectual property
produced as a result of this award shall be produced for the "public domain."
As such, The California Endowment, grantee, or any other party shall have a
nonexclusive irrevocable, perpetual, and royalty-free license to reproduce,
publish, copy, alter or otherwise use the intellectual property so produced.
1. Travel. Conference and Food Expenses. Grantee agrees to abide by its budget
for travel, conferences and food expenses as set forth in the budget submitted
with Grantee's Proposal.
K. Licensinll and Credentials. Grantee hereby agrees to maintain, in full force
and effect, all required governmental or professional licenses and credentials
for itself, its facilities, and for its employees and all other persons engaged in
work in conjunction with this award.
L. Manl\llement and Orllanizational ChanlleS. Grantee agrees to provide
immediate written notice to The California Endowment if significant changes
or events occur during the term of this award which could potentially impact
the progress or outcome of the grant, including, without limitation, changes in
Grantee's management personnel or losses of funding.

M. The California Endowment Letterhead and LOl:o. Use of The Califomia
Endowment letterhead or logo is prohibited without prior written consent from
the Communications Department of The Califomia Endowment.

Congratulations on receiving approval for funding of your program. We look forward to
working in partnership with you to create positive change and build healthy communities
throughout California.
We ask that you let us know what your public relations plans are. Members of The California
Endowment's Communications Department are available to provide communications support
to our grantees. As you know, through communication, journalists, readers, listeners and
viewers are informed about the role of philanthropy in health and the vitally important
contributions of nonprofit organizations in local communities and throughout California.
As you prepare to make an announcement of your grant through the news media, your internal
newsletter(s), the public, and other appropriate groups in the health and human services
sectors, we ask that you follow these guidelines:
· We ask that you announce your grant on{v qfter you have signed and returned your
grant agreement and discussed your announcement strategy with The Endowment's
Communications Department. This policy also applies to all future communication
pieces mentioning The California Endowment.
· If your organization is releasing an RFP (Request for Proposals) funded by a grant from
The California Endowment, that RFP must be reviewed by a member of The
Endowment's Communications Department prior to its issuance, and we do ask that The
Endowment's funding be acknowledged.
· Upon your grant agreement being signed and returned, a member of The Endowment's
Communications Department may call to discuss communications strategies, including
media announcements, development of media lists, having joint media contacts from The
Endowment and your organization on a press release, how to go about placing the release
in various media outlets, how to notify key government officials, etc. In all instances, the
