Synopsis of Previous Council action:
The City previously entered into an agreement with Standard Life Insurance on
November 15, 1971, to provide Term Life Insurance for certain City employees
and their dependents.
Recommended motion:
Approve the amendment to extend the term of agreement an additional year effective
December 1. 1984, with no change in rates.

RESOLUTION NO.
RESOLUTION OF THE CITY OF SAN BERNARDINO AUTHORIZING THE
EXECUTION OF AN AMENDMENT TO AGREEMENT WITH STANDARD LIFE INSURANCE
COMPANY OF PORTLAND, OREGON, EXTENDING THE TERM FOR AN ADDITIONAL
YEAR, EFFECTIVE DECEMBER 1, 1984.
BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL OF THE CITY OF
SAN BERNARDINO AS FOLLOWS:
SECTION 1. The Mayor of the City of San Bernardino is hereby
authorized and directed to execute on behalf of said City an
Amendment to Agreement with Standard Life Insurance Company of
Portland, Oregon, extending the term for an additional year,
effective December 1, 1984, a copy of which is attached hereto,
marked Exhibit "An and incorporated herein by reference as fully
as though set forth at length.
I HEREBY CERTIFY that the foregoing resolution was duly
adopted by the Mayor and Common Council of the City of San
Bernardino at a

AMENDMENT TO AGREEMENT
(life insurance)
THIS AMENDMENT TO AGREEMENT is entered into effective
December 1, 1984, between STANDARD LIFE INSURANCE COMPANY OF
PORTLAND, OREGON, "Standard Life", and the CITY OF SAN BERNARDINO,
a municipal corporation, "City".
The parties agree as follows:
1. Recitals.
(a) Standard Life and City entered into an agreement on
November 15, 1971, under which agreement term life insurance is
provided certain City employees and their dependents, and
accidental death and dismemberment insurance is provided by
Standard Life to participating employees.
(b) Standard Life and City now desire to amend the
agreement to provide for a one year extension of the term of the
agreement.
2. Term. The term of the agreement is hereby extended for
an additional one year period effective December 1, 1984, and
terminating November 30, 1985.
3. Other Provisions. All other terms, conditions and
covenants of the agreement, as amended, shall remain in full force
and effect.
IN WITNESS WHEREOF, the parties hereto have executed this
Amendment to Agreement on the dates set forth below.
