STAFF REPORT
Subject: Authorization to execute Contract Change Order No.1 with Sean Malek Engineering
& Construction, Inc.-Rialto Bridge at Twin Creek, Widen Bridge and Street Improvements
per Plan No. 9726.
Background: On February 15, 1999, the City entered into a contract with Sean Malek
Engineering & Construction, Inc. for the widening of Rialto Avenue Bridge and Street
Improvements at Twin Creek per Plan No. 9726.
During construction it was found that the existing bridge approaches had settled and that
certain changes to the design were needed to assure that the bridge widening will be safe,
functional, and provide a high quality of ride for motorists.
Contract Change Order No. 1 consists of the following items:

Item 1: Additional labor and equipment to remove unsuitable soil in area of $2,768.81
new approach slabs and to replace with cement treated base in order to
oreclude further settlement.
Item 2: Compensation for an additional 75 tons of AC Pavement at the bid $4,500.00
price of $60.00 per ton to bring the existing sunken road bed up to grade
for a smooth ride.
Item 3: Extra Joint Seal to replace existing failed joint seal between $185.00
existing approach slab and existinl! bridge.
Item 4: Imported borrow to replace unsuitable soil which was removed in $9,842.50
Item 1.
TOTAL COST $17,296.31
A copy of Change Order No. 1 is attached which will increase the contract total from
$399,969.50 to $417,265.81.
Construction of this project is complete and approval of this change order will allow Staff to
close out the project and make a fInal payment to the Contractor.
Financial Impact: SuffIcient funding for this Change Order is available in Account No. 129-
367-5504-7064 "Rialto Bridge at Twin Creek - Widen and Improve".
Contract Change Order No. 1 is the only change order for this project and the total cost is
within the $25,100 which was allotted for contingencies at time of contract award.
Recommendation: Staff recommends approval of Contract Change Order No.1.
