Synopsis of Previous Council Action:
July 6, 1999 Allocated $145,000 from the Traffic Systems Fund as part of the FY
1999-2000 Budget to fund a traffic signal and median at
Tippecanoe/Laurelwood.
August 2, 1999 Approved installation of temporary delineators for a period of 6 months
and approved installation of a traffic signal at the intersection of
Tippecanoe Avenue and Laurelwood Drive.
August 9,1999 Approved Plan No. 10012 for installation ofa traffic signal at the
intersection of Tippecanoe Avenue and Laurelwood Drive, and authorized
the Director of Development Services to advertise for bids.
January 10, 2000 Resolution 2000-6 adopted awarding construction contract to Signal
Maintenance Inc. in the amount of$81,260.00.
Recommended Motion:
That Contract Change Order No. One to the contract with Signal Maintenance Inc. be
approved for additional conduit, pull boxes and additional concrete R & R, with credit for a
change in conduit size, for a total Change Order amount of $5,855.00, increasing the contract
total from $81,260.00 to $87,115.00. ~

STAFF REPORT
Subject:
Authorization to execute Contract Change Order No. I with Signal Maintenance Inc.-
Installation of Traffic Signal at the Intersection of Tippecanoe A venue and Laurelwood A venue
per Plan No. 10012.
Background:
On January 10, 2000, the City entered a contract with Signal Maintenance Inc. for the
Installation of Traffic Signal at the Intersection of Tippecanoe A venue and Laurelwood Avenue
per Plan No. 10012.
At the time of bidding, it was recognized that some changes may be needed to the design to
insure consistency between the signal design and the planned realignment of Rosewood Drive
to Laurelwood Drive in connection with a redevelopment project presently under study by the
Economic Development Agency. The EDA project has progressed faster than expected,
therefore, Staff has revised the signal plan to accommodate the realignment, as much as
possible, within the existing right-of-way. The Contractor has provided a price for this
revision (see attached letter), and we believe this to be a fair and reasonable price.

A copy of Change Order No. I is attached which will increase the contract total from
$81,260.00 to $87,115.00.
Financial Impact:
Sufficient funding for this Change Order is available from the $145,000.00 budgeted in
Account No. 129-367-5504-7172 (Tippecanoe/Laurelwood-Install Traffic Signal and Median).
Recoimnendation:
Staff recommends approval of Contract Change Order No. 1.

Gentlemen:
. The revised plans received with revisions on 1/4100 have been reviewed and the following
determination and proposal are hereby submitted.
The cost for the additional conduit, pull boxes and additional concrete R & R is
Lump Sum $ 6,350.00.
In addition to the above a credit of $ 495.00 is due for the Change in conduit size for conduit
already shown as 2" and now being changed to 3".
The total additional cost for the changes made is LUMP SUM S 5,855.00.
If this is acceptable, please issue an appropriate Contract Change Order at your earliest
convenience.
