2 RESOLUTION OF THE CITY OF SAN BERNARDINO ESTABLISHING
CONDITIONS OF ELIGIBILITY FOR SPOUSES OF DECEASED CITY PERS
3 RETIREES FOR CONTINUED COVERAGE UNDER CITY'S COMPREHENSIVE
MEDICAL PLAN.
4 WHEREAS, it has been determined that the surviving
5 spouse of a retired City employee should have the option of
6 continuing to be covered by the City's Comprehensive Medical
7 Plan; and
8 WHEREAS, the Benefits Insurance Committee, comprised of
9 members from all of the bargaining units have agreed that the
10 surviving spouse of a deceased retired employee should have
11 the option of continuing coverage under the City's Compre-
12 hensive Medical Plan provided the cost of such coverage is
13 allocated to group's premium rate with no cost to the City.
14 NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND COMMON
15 COUNCIL OF THE CITY OF SAN BERNARDINO AS FOLLOWS:
16 SECTION 1. The surviving spouse of a deceased City PERS
17 retiree shal_1 have the option of continuing coverage under
18
the City's Comprehensive Medical Plan, provided the City
19
receives, on a monthly basis as a personal expense to the
20
individual, the then current subscriber rate under the Plan.
21
Failure to pay the monthly subscriber rate for a period of
22
thirty days shall automatically cancel the coverage and shall
23
not be re-instated.

SECTION 2. Surviving spouses of deceased City PERS
25
retirees who have previously exercised the option for cover-
26
age shall be permitted to continue such coverage, provided

the City receives on a monthly basis as a personal expense to

2 Plan. Failure to pay the monthly subscriber rate for a
3 period of thirty days shall automatically cancel the cover-
4 aqe.
5 SECTION 3. Surviving spouses of deceased City PERS
6 retirees who have applied for continued coverage under the
7 City's Comprehensive Medical Plan, but whose applications
8 have not been accepted, shall be permitted to reinstate
9 coverage within thirty days following the effective date
10 hereof, provided the City receives, on a monthly basis as a
11 personal expense to the individual, the then current sub-
12 scriber rate under the Plan. Failure to pay the monthly
13 subscriber rate for a period of thirty days shall auto-
14 matically cancel the coverage.
15 SECTION 4. Such surviving spouses shall not be eligible
16 for coverage under the following:
17 1. Upon remarriage or death.
18 2. In the event such spouses have coverage under
19 another primary health plan.
