STAFF REPORT
SUBJECT
Resolution of the Mayor and Common Council authorizing the execution of a contract
between the City of San Bernardino and the County of San Bernardino for Public Health
Nursing services in the Operation Phoenix Target Areas.
BACKGROUND
On November 19, 2007, the Mayor and Common Council approved the expansion of the
Operation Phoenix Neighborhood Initiative, authorizing the Director of Finance to amend
the FY 07-08 Adopted Budget to reflect the cost of expanding the Operation Phoenix
Neighborhood Initiative for the remainder ofFY 07-08.
Included in the expansion was an agreement between the City of San Bernardino and the
San Bernardino County Department of Public Health for Public Health Nursing services
for the Operation Phoenix target areas for the period of January I - June 30, 2008. A
total of $45,877 was approved for the reimbursement of Public Health Nursing services
as part of the November 10,2007 Council action.
The Public Health Nursing Program is designed to ensure the health and safety of San
Bernardino County residents by accomplishing the following:
· Promoting and encouraging healthy behaviors
· Assuring the quality and accessibility of health services
· Educating and empowering people about health issues
· Enforcing laws and regulations that protect health and safety
· Monitoring the health status of the community
Specific program examples include:
. Alcohol and Drug Abuse Prevention
. Tobacco use reduction
· Aids and Sexually Transmitted Disease Control
· Maternal, Child and Adolescent Health Services
· Family Support Services
These services would not necessarily be focused in the city and Operation Phoenix areas
without the City's financial participation. All Public Health programs are categorically
funded.
Since the first team inspection done on June 23, 2006, through the inspection done on
February 27 2008, Public Health Nurses evaluated 198 households with 772 members in
the households. Currently, we have active cases with 285 household members. These
inspections have allowed public health to provide the community with essential health
related information and connection to resources that would otherwise be ignored. We
have been able to get into these homes where the families were either unable, due to lack
of knowledge, or afraid because of the unsafe neighborhood to try and access services.

Attached is information from the County Public Health Department describing several
scenarios whereby the Public Health Nurse's services were utilized with beneficial
results.
FINANCIAL IMPACT
$45,877 will be reimbursed to the County for Public Health Nursing from Account
Number 001-076-5505.

COUNTY PUBLIC HEALTH REPORT
Since the first sweep occurred on June 23, 2006, through the sweep occurring on February 27,
2008 , the Public Health Nurses (PHN) participated in 6 block parties, evaluating approximately
3000 community members. They did blood pressure screening and consultations on various
medicaVdental and behavioral health issues. Public Health provided educational materials
geared to the lay population.
During one ofthe block parties, a 15 year old pregnant teen approached the booth with her
mother. The PHN asked her about her pregnancy and discovered that she had only been to the
physician 2 times. She was 7 months along. The PHN obtained her demographics, and got
permission from the teen and her mother to refer to the PH Perinatal Adolescent Life Section
which manages the pregnant and parenting teen. She enrolled and received appropriate health
care. As a caveat, during the recent fire evacuations, the teen recognized the PHN from the OP
block party. She was with her mother and her I year old child. She expressed gratitude for the
help she and her daughter received. She stated she was going to school and would graduate in
2008.
Also at a block party in 2007, a Hispanic father of 3 stopped by the booth for a blood pressure
check. He was with his wife who stated he is diabetic but not taking care of himself. He had not
been to a physician for years. His blood pressure was elevated. The PHN spent a great deal of
time educating him and his wife about the importance of managing his disease. He did not have
insurance for himself as his job did not offer any. A referral was made to the LLUMC SAC
clinic for follow up on his medical condition. The OP PHN followed up one month later to find
out that he did go to the clinic, and was now on a diabetic regime. The father expressed that
without the intervention of the PHN, most likely he would either be dead or out of job and
unable to provide for his family.
On a recent sweep, a family of 7 was living marginally in a 2 bedroom apartment in the East OP
area. The PHN discovered that an older gentleman was suffering from medical neglect. His
conditions prohibited him from sleeping supine. He was only able to carry out minimal daily
routine activities. His only insurance was medical MIA (Medically Indigent Adult) because his
SSI was pending. His wife also suffered numerous medical conditions which limited her ability
to care for the children and her spouse. The PHN helped the family with referrals to the County
McKee clinic. The family was displaced from their vermin infested apartment by Code
Enforcement and put in a motel. They subsequently contacted the PHN to report that the male
householder could not sleep flat in the motel. The PHN contacted Adult Protective Service
hotline to report the issue. APS provided him with a wedge for sleeping. He was also assisted
with obtaining MediCal. Without the intervention of the PHN, this male would have probably
ended up in the hospital, putting an even greater strain on his family situation.

WHEREAS, the Mayor and Common Council approved the expansion of the Operation
Phoenix initiative; and
WHEREAS, the Finance Director was authorized to amend the Fiscal Year 07-08
Budget to reflect the costs associated with the expansion; and
WHEREAS, the County of San Bernardino has agreed to provide Public Health
Nursing Services to the Operation Phoenix expansion areas as set forth in that standard contract
between the County of San Bernardino and the City (Agreement).
NOW, THEREFORE BE IT RESOLVED BY THE MAYOR AND COMMON
COUNCIL OF THE CITY OF SAN BERNRDINO AS FOLLOWS:
SECTION 1. The City Manager is hereby authorized to execute the formal Agreement
attached hereto as Exhibit A.
SECTION 2. The total reimbursement amount shall not exceed $45,877.
SECTION 3. This Resolution is rescinded if the parties to the Agreement fail to
execute within sixty (60) days of the passage of this Resolution.

This agreement is made and entered into by the City of San Bernardino, hereinafter referred to as City, and the County of
San Bernardino, a political subdivision of the State of California, hereinafter referred to as County. The San Bernardino
County Department of Public Health will hereinafter be referred to as Department.
WHEREAS, the above named parties desire to set forth in writing the specific terms and conditions mutually agreed to
regarding nursing services for the purposes authorized by Sections 49452, 49452.5, 49454 and 49455 of the Education
Code.
NOW THEREFORE, it is mutually agreed and covenanted that:
1. The Department shall provide nursing services for the City of San Bernardino, Operation Phoenix as listed in
Attachment A to this agreement. Hours of service to be rendered to City shall be performed as indicated in Attachment
A to this agreement. Attachment A is hereby made a part of this agreement.
2. City will reimburse the Department at the rate of $45.00 an hour, for 832 hours, for a total of $37,440 for nursing services
and $4,992 for nursing supervision with corresponding benefits of $2,368 for a total personnel cost of $44,800 as listed in

Attachment B to this agreement. Costs for services and supplies will be $1,077 as listed in Attachment B
Attachment B is hereby made a part of this agreement.
3. Reimbursement for all services referenced herein shall be due upon receipt of an invoice stating the hours of services
rendered and the costs incurred thereby as listed in Attachment C; said invoice to be rendered to. City by the
Department on or about the following date(s): April 30, 2008 and July 31, 2008. Attachment C is hereby made a part of
this agreement.
4. The Department or the City may request changes to the contracted hours through a written amendment. Otherwise,
alteration or variation of the terms of this agreement shall be invalid unless made in writing and signed by the parties
hereto. No oral understanding or agreement not incorporated herein shall be binding on any of the parties hereto.
5. City agrees to indemnify, defend and hold harmless the Department, the County and their authorized agents, officers,
volunteers and employees against any and all claims or actions arising from the City's acts or omissions and for any
costs or expenses incurred by the Department, County or City on account of any claim therefore.
6. The County agrees to indemnify and hold harmless the City and its authorized agents, officers, volunteers and
employees against any and all claims or actions arising from County's negligent acts or omissions and for any costs or
expenses incurred by the Department, County or City on account of any claim therefore.
7. The period of this agreement shall be from January 1, 2008 through June 30, 2008. This agreement may be terminated
by either party upon thirty (30) days written notice to the other. The Public Health Director shall exercise full County
rights under the terms of this agreement. In the case of such termination, the Department will invoice the City for costs
associated with activities performed in accordance with this agreement prior to the effective date of termination.

Scope of Work for
PUBLIC HEALTH NURSE
Operation Phoenix
Definition
Under the general direction of the Supervising Public Health Nurse, the Public Health Nurse
(PHN) position will perform a variety of public health nursing duties focused on the concepts of
health care coordination for the population served by Operation Phoenix (OP). The PHN is
expected to exercise independent and professional judgment in assessing the complex medical,
developmental, mental health, and dental needs and problems faced by children/adolescents,
medically fragile elderly, their families and other related individuals. Additionally the PHN must
have a thorough and detailed knowledge of the laws, regulations, and procedures governing the
various programs administered at the county level and working familiarity of health care
resources in the community.
Duties include but are not necessarilv limited to the followiol!:
A. Identify and track each referral from the various agencies operating in the OP service
area.
B. Participate in OP Street Team sweeps along with the other agencies.
C. Use skilled medical professional expertise; make appropriate referrals to agencies as
needed for families and individuals in the OP area.
D. Using skilled medical professional expertise, provide support and professional
consultation to agencies on the OP Street Team on issues of medical, dental,
developmental and mental health.
E. Monitor the treatment progress of the OP referrals, and advise the referent of the
progress.
F. Participate in all the OP functions including but not limited to OP Street Team meetings,
block parties, expansion events etc.
G. Provide support to the OP Street Team as requested including consultation on matters
relating to medical (includes issues of domestic violence), dental, developmental,
nutritional issues. Participate in multi-disciplinary case consultation team to provide
explanations and recommendations regarding preventive and acute health related
problems with appropriate interventions.
H. Coordinate provision of health education, Public Health Nursing/Case Management, and
specialized follow-up to substance exposed children in the Operation Phoenix area.
L Provide confidential counseling and referral for treatment regarding Sexually
Transmitted Diseases and other health related issues.
Ouality Manal!ement
A. Maintain data log reporting the cases received, tracking outcomes, monitoring to see that
health care problems receive appropriate and timely attention, and describing the level of
client compliance.

B. Participate in team meeting to evaluate and make recommendations in the development,
implementation, and data collection for quality assurance/quality improvement plan.
C. Evaluate effectiveness of the PHN role in relation to the OP project.
Location and hours:
A. The PHN will be housed at the San Bernardino Code Enforcement Office located at 210
North "E" Street San Bernardino.
B. The PHN will be provided the tools for managing the cases, which include, but are not
limited to: desk, telephone, computer with MS Office installed, access to a printer, copier
and fax and other office supplies.
C. The PHN will work .4 FTE at this time. This will be a contract position with San
Bernardino Department of Public Health.
D. The PHN will work two 8 hour weekdays for the period January 1, 2008 through June 30,
2008.

developmental and mental health.
E. Monitor the treatment progress of the OP referrals, and advise the referent of the
progress.
F. Participate in all the OP functions including but not limited to OP Street Team meetings,
block parties, expansion events etc.
G. Provide support to the OP Street Team as requested including consultation on matters
relating to medical (includes issues of domestic violence), dental, developmental,
nutritional issues. Participate in multi-disciplinary case consultation team to provide
explanations and recommendations regarding preventive and acute health related
problems with appropriate interventions.
H. Coordinate provision of health education, Public Health Nursing/Case Management, and
specialized follow-up to substance exposed children in the Operation Phoenix area.
I. Provide confidential counseling and referral for treatment regarding Sexually
Transmitted Diseases and other health related issues.

A. Maintain data log reporting the cases received, tracking outcomes, monitoring to see that
health care problems receive appropriate and timely attention, and describing the level of
client compliance.

A. The PHN will be housed at the San Bernardino Code Enforcement Office located at 210
North "E" Street San Bernardino.
B. The PHN will be provided the tools for managing the cases, which include, but are not
limited to: desk, telephone, computer with MS Office installed, access to a printer, copier
and fax and other office supplies.
C. The PHN will work .4 FIE at this time. This will be a contract position with San
Bernardino Department of Public Health.
D. The PHN will work two 8 hour weekdays for the period January 1,2008 through June 30,
2008.

This agreement is made and entered into by the City of San Bernardino, hereinafter referred to as City, and the County of
San Bernardino, a political subdivision of the State of California, hereinafter referred to as County. The San Bernardino
County Department of Public Health will hereinafter be referred to as Department.
WHEREAS, the above named parties desire to set forth in writing the specific terms and conditions mutually agreed to
regarding nursing services for the purposes authorized by Sections 49452, 49452.5, 49454 and 49455 of the Education
Code.
NOW THEREFORE, it is mutually agreed and covenanted that:
1. The Department shall provide nursing services for the City of San Bernardino, Operation Phoenix as listed in
Attachment A to this agreement. Hours of service to be rendered to City shall be performed as indicated in Attachment
A to this agreement. Attachment A is hereby made a part of this agreement.
2 City will reimburse the Department at the rate of $45.00 an hour, for 832 hours, for a total of $37,440 for nursing services
and $4,992 for nursing supervision with corresponding benefits of $2,368 for a total personnel cost of $44,800 as listed in

3. Reimbursement for all services referenced herein shall be due upon receipt of an invoice stating the hours of services
rendered and the costs incurred thereby as listed in Attachment C; said invoice to be rendered to. City by the
Department on or about the following date(s): April 30, 2008 and July 31, 2008. Attachment C is hereby made a part of
this agreement.
4. The Department or the City may request changes to the contracted hours through a written amendment. Otherwise,
alteration or variation of the terms of this agreement shall be invalid unless made in writing and signed by the parties
hereto. No oral understanding or agreement not incorporated herein shall be binding on any of the parties hereto.
5. City agrees to indemnify, defend and hold harmless the Department, the County and their authorized agents, officers,
volunteers and employees against any and all claims or actions arising from the City's acts or omissions and for any
costs or expenses incurred by the Department, County or City on account of any claim therefore.
6. The County agrees to indemnify and hold harmless the City and its authorized agents, officers, volunteers and
employees against any and all claims or actions arising from County's negligent acts or omissions and for any costs or
expenses incurred by the Department, County or City on account of any claim therefore.
7. The period of this agreement shall be from January 1, 2008 through June 30, 2008. This agreement may be terminated
by either party upon thirty (30) days written notice to the other. The Public Health Director shall exercise full County
rights under the terms of this agreement. In the case of such termination, the Department will invoice the City for costs
associated with activities performed in accordance with this agreement prior to the effective date of termination.

PUBLIC HEALTH NURSE
Operation Phoenix
Definition
Under the general direction of the Supervising Public Health Nurse, the Public Health Nurse
(PHN) position will perform a variety of public health nursing duties focused on the concepts of
health care coordination for the population served by Operation Phoenix (OP). The PHN is
expected to exercise independent and professional judgment in assessing the complex medical,
developmental, mental health, and dental needs and problems faced by children/adolescents,
medically fragile elderly, their families and other related individuals. Additionally the PHN must
have a thorough and detailed knowledge of the laws, regulations, and procedures governing the
various programs administered at the county level and working familiarity of health care
resources in the community.
Duties include but are not necessarilv limited to the followiUl!:
A. Identify and track each referral from the various agencies operating in the OP service
area.
