Synopsis of Previous Council action:
Adoption of Resolution No. 90-66 on March 5, 1990, authorizing the execution of an
agreement relating to implementation of a long term disability program for manage-
ment and confidential employees.

CITY OF SAN BERCARDINO - REQUESTQOR COUNCIL ACTION
STAFF REPORT
On March 5, 1990, Council adopted Resolution No. 90-66 authorizing
the execution of an agreement with standard Insurance Company
relating to implementation of a long term disability (LTD) program
for Management and Confidential employees.
We are recommending replacement of the Standard Policy at no
additional cost to the City. The policy covers 120 Management/
Confidential and 73 Mid-Management employees. The city pays the
premiums for Management/Confidential while Mid-Management employees
pay their own premiums. The current rates are $23.90/member/month
for Management/Confidential and $23.90/member/month for Mid-
Management. The new rate is $23.76 per employee for the UNUM
policy. The UNUM policy increases the benefits of Long Term
Disabili ty from 1 year to 5 years. However, the program is
designed to provide co-ordination of benefits and return employees
to the workplace through incentives. There is no increase in cost
to the City and will not impact the General Fund.
The policy will become effective 1/1/92, and may be terminated at
any time by giving prior written notice to UNUM Insurance company.
In your packets is a Resolution to adopt the complete LTD policy.
If the Resolution is delayed until the next Council meeting, we
could have an increae in premium by the carrier.

RESOLUTION OF THE CITY OF SAN BERNARDINO AUTHORIZING THE
PTION OF UNOM INSURANCE COMPANY LONG TERM DISABILITY POLICY FOR
AGEMENT/CONFIDENTIAL AND MID MANAGEMENT EMPLOYEES.
BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL OF THE CITY OF
SAN BERNARDINO AS FOLLOWS:
WHEREAS, the Council adopted Resolution No. 87-155 on May 18,

1987, relating to implementation of a long term disability program

for management and confidential employees,
WHEREAS, Council later authorized the addition of Mid
anagement employees to that policy,
WHEREAS, UNOM Insurance Company has prepared a comprehensive
document incorporating all relevant parts of the application
or insurance adopted by Resolution No. 87-155 plus subsequent
mendments to that application as described in the staff report.
NOW THEREFORE BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL
F THE CITY OF SAN BERNARDINO that the UNUM Insurance Company LTD
olicy be adopted, replacing the present policy with Standard Life
This Resolution shall apply only to the Managementl
onfidential and Mid-Management city employees.
I HEREBY CERTIFY that the foregoing Resolution was duly
