HomeMy WebLinkAbout1986-195 1 RESOLUTION NO. 86-195 2 RESOLUTION OF THE CITY OF SAN BERNARDINO ESTABLISHING INTERIM CONDITIONS OF ELIGIBILITY FOR SPOUSES OF DECEASED CITY 3 PERS RETIREES FOR CONTINUED COVERAGE UNDER CITY'S COMPREHENSIVE MEDICAL PLAN. 4 5 WHEREAS, a study is being conducted relative to the 6 surviving spouse of a retired City employee having the option of 7 continuing to be covered by the City's Comprehensive Medical 8 Plan; and 9 WHEREAS, in order not to jeopardize individuals presently 10 covered and others desiring to avail themselves of the option to 11 continue coverage, pending the results of the study, it is 12 considered necessary 'to establish an interim policy with respect 13 to continued coverage for such individuals. 14 NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND COMMON 15 COUNCIL OF THE CITY OF SAN BERNARDINO AS FOLLOWS: 16 SECTION 1. An interim policy delineating eligibility 17 for spouses of deceased City PERS retirees for continued coverage 18 under the City's Comprehensive Medical Plan is hereby established 19 and shall continue in effect for a period of three years, or until 20 the Mayor and Common Council shall adopt a revised policy, 21 whichever date should first occur. 22 SECTION 2. The surviving spouse of a deceased City PERS 23 retiree shall have the option of continuing coverage under the 24 City's Comprehensive Medical Plan, provided the City receives, on 25 a monthly basis as a personal expense to the individual, the then 26 current subscriber rate under the Plan. Failure to pay the 27 monthly subscriber rate for a period of thirty days shall 28 automatically cancel the coverage. 1 SECTION 3. Surviving spouses of deceased City PERS 2 retirees who have previously exercised the option for coverage 3 shall be permitted to continue such coverage, provided the City 4 receives on a monthly basis as a personal expense to the 5 individual the then current subscriber rate under the Plan. 6 Failure to pay the monthly subscriber rate for a period of thirty 7 days shall automatically cancel the coverage. 8 SECTION 4. Surviving spouses of deceased City PERS 9 retirees who have applied for continued coverage under the City's 10 Comprehensive Medical Plan, but whose applications have not been 11 accepted, shall be permitted to reinstate coverage within thirty 12 days following the effective date hereof, provided the City 13 receives, on a monthly basis as a personal expense to the 14 individual, the then current subscriber rate under the Plan. 15 Failure to pay the monthly subscriber rate for a period of thirty 16 days shall automatically cancel the coverage. 17 SECTION 5. Such surviving spouses shall not be eligible 18 for coverage under the following: 19 1. Upon remarriage. 20 2. In the event such spouses have coverage under 21 another primary health plan. 22 3. A three-year period has elapsed from the effective date 23 of this resolution. 24 I HEREBY CERTIFY that the foregoing resolution was duly 25 adopted by the Mayor and Common Council of the City of San 26 Bernardino at a reaular meeting thereof, 27 he ld on the 2nd day of June , 1986, by the 28 following vote, to wit: 2 AYES: Council Members Reilly.
