STAFF REPORT
SUBJECT:
Authorize the establishment of Emergency Medical System Coordinator position in the
Fire Department.
BACKGROUND:
On January 29, 2001, the Mayor and Council authorized the Fire Department to enter
into an agreement with American Medical Response (AMR) to provide Enhanced Pre-
Hospital Paramedic Service. This public/private partnership has allowed the City to staff
all eleven engines with paramedics and utilize both the City and AMR's medical
equipment, resulting in improved patient care. This partnership also allowed AMR to
eliminate 1.5-ambulance units within the City, resulting in cost savings of $370,201.20
annually. AMR will share this cost savings with the City as follows: 60% [$222,120.72]
to the City and 40% [$148,080.48] to AMR.
For calendar year 2000, the Fire Department responded to 18,247 emergency medical
aid calls. This represents 85% of the total emergency call volume. The City's
emergency medical service (EMS) is an important segment of the Fire Department's
service delivery. Additionally, there are a number of related activities that needs to be
managed in the EMS area. Some of those areas are: coordination of and delivery of
training for all paramedics and emergency medical technicians, quality
assurance/quality improvement programs, medical supply inventory programs, scope of
practice/protocols, billing trouble shooting, etc.
One of the components of the program previously approved by Mayor and Common
Council involved the creation of an Emergency Medical System Coordinator position,
which would be responsible for oversight of the EMS program.
Currently there is no one on the Fire Department staff that can devote their full efforts to
the EMS program. In order for the EMS program to be successful, there needs to be an
individual assigned to manage the program full-time. The duties of the position will
include:
. Working with emergency operations staff and outside agencies to effectively
delivery EMS services.
. Providing and/or coordinating necessary training for all department
paramedics.
. Representing the Department on EMS related committees and/or groups with
external agencies.
. Ensuring that the Fire Department is meeting all governmental regulations
and standards.
. Keeping the department updated on new processes and technologies in the
area of EMS.
FINANCIAL IMPACT:
In an effort to establish a competitive salary, a survey of Fire agencies was conducted.
The development of these types of programs is a relatively new trend in Fire services
for this reason the agencies surveyed included some that are not typically used by the
City of San Bernardino. They included:

Based on the results of the survey data we are recommending that the monthly salary
for this position be set at Range 4478, $4,816 - $5,854. The recommended range
places the position in the Management group, which reflects the responsibilities as a
program coordinator. The implementation of the Classification and Compensation will
not affect this position

The revenues generated from this contract will be included in the Fire Department's
General Fund budget to pay for the cost assisted with providing the increased
paramedic services related to this program, including the proposed EMS Coordinator
position, as well as miscellaneous related cost which will be identified during the annual
budget process by the Fire Department.

t)'\ EMERGENCY MEDICAL SERVICES COORDINATOR
JOB DESCRIPTION
Under administrative direction develops, organizes, directs and administers the Fire
Department's Emergency Medical Services activities within the Division of Emergency
Operations, which includes both Basic Life Support (BLS) and Advanced Life Support (ALS)
functions. Monitors the daily and weekly activities of administrative and support staff to ensure
compliance with program performance reqnirements. Conducts and coordinates long-term and
short-term planning processes. Interacts with a wide variety of constituency groups, including
Inland Counties Emergency Medical Authority (ICEMA) and American Medical Response
(AMR), in order to maintain high quality services, and performs related work as reqnired.
REPRESENTATNE DUTIES
Provides courteous and expeditious customer service to the general public and City department
staffs.
Develops, implements, administers and evaluates the Emergency Medical Services Program.
Develops short term and long term objectives, develops and implements program operating
policies and procedures, and organizes, prioritizes, assigns and assesses all program
administrative duties. Evaluate legislation, regulations and local protocols that may impact
Paramedic, EMT, or other department EMS programs. Make recommendations in writing
regarding suggestions, changes, support or opposition to proposed changes.
Work closely with the department Training Officer to ensure proper EMS training, continuous
quality improvement, drug and equipment use, and protocol review. Develops performance
measures and training standards to ensure quality and efficiency of services. Monitor all EMS
programs, operations and personnel for compliance with State and Local laws, regulations,
protocols, licensure and accreditation requirements; and assist EMT's and Paramedics with re-
certification needs. Develop, implement, educate and update the exposure control program,
polices and procedures to include exposure reports, and blood borne/air borne pathogen training.
Establish and maintain the EMS budget as needed.
Routinely adheres to and maintains a positive attitude towards City and Department goals, and
performs related work as required.
MINIMUM OUALIFICATIONS
Bachelor's or Master's degree in Business or Public Administration, Business Management,
Nursing or Health Administration or a related field and two (2) years of responsible
administrative and supervisory experience in emergency medical services.
Up to two (2) additional years of specified experience may substitute for up to two (2) years of
the required education on the basis of thirty (30) semester units being equivalent to one (1) year
of experience. Certification as a paramedic and/or registered nurse is highly desirable.
Possession of a valid Class "C" California Driver's License is required.
GENERAL OUALIFICATIONS
Knowledge of:
Principles and practices of office administration, budgeting, supervision, training and public
relations;
Modem office equipment and procedures including basic computer skills;
Proper English usage, spelling, grammar, punctuation and report writing;
Business practices as they relate to the operation of a fire service based emergency medical
services program.
Ability to:
See in the normal visual range, with or without correction;
Hear in the normal audio range, with or without correction;
Plan, organize and direct a comprehensive EMS program;
Plan and direct the work of, supervise and train EMS supervisory and support staff personnel;
Understand and carry out oral and written instructions;
Express ideas clearly and concisely, orally and in writing;
Establish and maintain effective relationships with those contacted in the course of work.
ORGANIZATIONAL RELATIONSHIPS
The class of Emergency Medical Services Coordinator is a program director within the Fire
Department. Supervision is received from the Fire Chief or his/her designee.
