BACKGROUND On November 6, 2006, a contract was issued to JM Builders, Inc. in the amount of $785,000, for the construction of the Perris Hill Senior Center Expansion. After the start of this project, the following changes to the scope needed to be made: Change Order Number One: Approval of various field instructions This Change Order is to approve various field instructions given to the contractor to keep the project moving along. These six items were reviewed by the contractor, the architect and Facilities Management before authorization to proceed was given. Item: Change to Contract Amount . I. Remove and replace dry rot lumber and plywood $880.00 2. Furnish and install roof drains in the roof of the addition $8,119.00 3. Delete all work in rooms 105 & 108 ($8,213.00) 4. Furnish and install power door operators on the existing restroom doors $10,683.00 5. Furnish and install gas piping to the fireplace $906.00 6. Relocate gas mete.r line $446.00 Total Change to Contract Amount: $12,821.00 e e e These changes were not identified prior to the start of construction and the costs are consistent with the Standard Specifications for Public Works Projects (Greenbook). Staff recommends approval of this change order. SUMMARY OF COSTS The revised project costs are as follows: Item Construction Cost Change Order No. One Revised Total Cost Percent $785,000.00 100.0% $12,821.00 1.6% $797,821.00 101.6% FINANCIAL IMPACT There is sufficient funding for this project in Account No. 243-363-5504-7494 - Perris Hill Senior Center Expansion, to cover this change order. RECOMMENDA nON That the City Manager be authorized to execute Change Order No. One to the contract with JM Builders, Inc. for changes in the scope of work at Perris Hill Senior Center Expansion in the amount of$12,821.00.
