2 A RESOLUTION OF THE MAYOR AND COMMON COUNCIL OF THE CITY
OF SAN BERNARDINO PROVIDING FOR EMPLOYEE EVALUATION REPORTS
3 FOR ALL EMPLOYEES OF SAID CITY EXCEPT THOSE EMPLOYEES SERVING
UNDER OR IN THE DEPARTMENT OF ELECTED OFFICIALS, AND REPEALING
4 RESOLUTION NO. 6667.
5 BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL OF THE CITY
OF SAN BERNARDINO, AS FOLLOWS:

SECTION ONE: Each department head or the appointing power
shall prepare or cause to be prepared and shall submit to the
Personnel Section of the Administrative Department of said City
an Em~oyee Evaluation Report for each employee in his depart-
.ent or under his supervision.
SECTION TWO: A sample copy of the Employee Evaluation Re-
port which is attached hereto and marked Exhibit "An, is hereby
referred to and made part hereof as fully as though set out at
length herein and is hereby approved.
SECTION THREE: If the appointing power is not the actual
rater preparing an Employee Evaluation Report, he shall be the

reviewer of said report and shall sign said report as Reviewer.
The supervisor who actually prepared the Employee Evaluation
Report shall sign said report as Rater.
The department head,
when functioning as Reviewer, may discuss a report with the
~ater and the employee, but he cannot change the report with-
out the concurrence of the Rater. If he recommends changes
with which the Rater agrees, the Rater either makes the changes
or prepares a new report. If he recoMMends changes with which
the Rater does not agree, the Reviewer will indicate his rating
of each factor on which there is disagreement and mark each
such rating with his initials. He also may add his co..ents
to those of the Rater and initial such co..entll.
SECTION FOUR: One Employee Evaluation Report for each
employee shall be submitted on or before a date designated by
the City Administrator once each year; provided, that in

addition to said yearly EMployee Evaluation Report, the depart-
Ment head or appointing power shall subMit for each probationary
eMployee and for each provisional eMployee a report within
three Months after the appointMent of said eMployee and an
additional Eaployee Evaluation Report within five Months after
such appointMent. Each employee shall review, sign and date
each report prepared concerning his perfor.ance since this is
a progress report to the eMployee, letting hOM know how well
he is doing his job and helping hiM orient his efforts effec-
tively. Copies of each of the reports mentioned in this Sec-
tion shall be filed in the office of the Chief Examiner and
Secretary of the Civil Service Board.
SECTION FIVE: Each E.ployee Evaluation Report shall be
considered by the Civil Service Board only for purposes in
connection with disciplinary proceedings or exaMinations for
proMotional applicants, but such report shall be considered by
the Mayor and Co..on Council, departMent head or appointing
power for any perMissible personnel purpose.
SECTION SIX: The provisions of this Resolution shall not
be applicable to eMployees serving under or in the departMent
of elected officers.
SECTION SEVEN: Each Employee Evaluation Report shall be a
confidential record and its contents shall be kept and reMain
confidential to and frOM other employees or persons not author-
ized to review and exaMine the saMe or to hear the contents
thereof.
SECTION EIGHT: Resolution No. 6667 is hereby repealed.
I HEREBY CERTIFY that the foregoing Resolution was duly
