Application and a proposal is hereby made to the Local Agency Formation Commission of the County of San Bernardino for the annexation of uninhabited territory located south of Highland Avenue, east of Sterling Avenue, and the detachment from County Service Area No. 38, Improvement Zone G 14 of CSA No. 38, County Service Area No. 70, Improvement Zone A of CSA No. 70, 15 and CSA Sl-l, as outlined in the justification form which is Exhibit "A" and 16 the plan and the narrative which is Exhibit "B" to the resolution and is by 17 this reference incorporated herein as though fully set forth. 18 SECTION 2. Vacant to the South, businesses North, East and West, 15-7983478 Rev. 5/77 Page 3 of 4 NARRATIVE FOR PROVn'IKG SERVICES FOR A PROPOSED ANNEXATIO: C TO THE CITY OF SAN BERNARDINO, LOCATED SOUTH OF HIGH- LAND AVENUE, EAST OF STERLING AVENUE The ~~yor, and Members of the Common Council of the City of San Bernardino agrees and desires to annex the above described uninhabited territory to the City of San Bernardino, for the following reasons: 1. The territory proposed to be annexed is encompassed on three sides by the corporate limits of said City, and whereas this adjacent City area is owned by a majority of those owners requesting annexation of their additional properties now lying within the County. 2. The incorporation of this territory will help contribute to ~nd facilitate the orderly growth and development of the existing City area and the territory proposed to be annexed. 3. This annexation will also help to provide a uniform and orderly manner of growth, planning and zoning of lands, and the subdivision of lands therein g.ch a manner most conducive to the welfare of said City and said uninhabited territory. The City of San Bernardino has planned for to the area under consideration for annexation. agreements with the County this area is already the City of San Bernardino. providng all typical City services Currently through contractural receiving the services provided by The following is an enumeration and description of services which will be ex- tended to the affected territory, which describes the level and range of suc~ ser- vices that will be provided, as well as the feasibility of extending such services and any improvements that will be required by the City, along with a description of when the services will commence and the financing of such improvements and services and any condition imposed by the City. 1. Planning, Zoning, Code Enforcement and Building Inspection - 'The City sha.!.l provide these services in conformance with appropriate government codes and City Ordinances as well as following the City's General Plan for zoning. This function is supported under the City's tax rate. 2. Parks and Recreation - The area proposed for annexation will !,ave no impact on park or recreational facilities or programs, and no additional service will be required as a result of the annexation. Recreation services available to this area are presently provided through the programs and recreational activities currently conducted at Perris Hill Park, approximately one mile west of this proposal, and facilities at Arden Farm approximately 1/2 ndle Southeast thereof. The cost to the taxpayer within the proposed annexation will be collected through the property taxes, and by required fees charged to those individuals who wish to participate in City sponsored recreational activities where charges and fees are applicable. -1- ,'" 3. Library Service - The City presently serves the area <lith two (2) branch libraries. Coddington Branch Library, lGcated at 1003 E. Highland Avenue, is the closest of the two; while RO<le Branch Library, at 108 E. Harshall Blvd., is the alternate branch. It is important to note that the resources of the entire library system are readily made available to the proposed annexation, including the DOImtown Central Library. 11 ." Public Safety, Police - The San Bernardino Police Department is authorized a complement of 199 sworn officiers and approximately 63 civilians not counting 53 school crossing guards. The Patrol Division maintains a 12 Beat or section split of the City. The Patrol Division presently averages 15 Police Units on Patrol 24 hours a day, counting Supervisory and Traffic personnel. The Detective Division has 30 sworn officers and works from 8:00 a.m. to 5:00 p.m. daily, Honday through Friday. The Services Division maintains all records and identification and provides support services to the other t<To divisions on a 24 hours a day basis. The only cost for these Police Services is property taxes to the City. 5. Public Safety, Fire, Paramedic and Rescue - The City of San Bernardino is currently providing fire protection to the proposed annexation area under CSA 038, calling for joint responses with the California Division of Forestry. Fire protection service which will continue upon annexation and the affected residents will be pro- vided with a Class 3 rated Fire Department with ten (10) different fire stations to respond. The closest of these is Station No. 6 located at 1920 Del Rosa Avenue. A t)~ical structural fire on the first alarm deploys two (2) engine companies, one (1) ladder truck company, a paramedic squad and one (1) Battalion Chief. In addition to fire protection, the City also has a minimum of three (3) paramedic units on duty 24 hours a day, each unit has two (2) paramedics each, giving a total of six (6) paramedics on duty at all times. Public Safety, Arebulance Service - The City of San Bernardino under Ordinance No. 1987 franchise to Courtesy Ambulance Company as the only medical transportation carrier within the City limits. The rates are established by the above ordinance and said 'fees are paid by the user as set forth below. (1) Ambulance rates: (a) Response to a routine non-emergency call,$45.00; non-routine emergency call, $10.00 additional. (b) Each mile or fraction of a mile $2.00. (c) ,On scene time $.50 per minute for calls requiring emer- gency response. (d) Night service charge 7:00 p.m. to 7:00 a.m., $7.50 (e) Response to a call for ambulance not used, $15.00. (f) For each additional patient, 50% over charge for one patient. -2- (g) Carrier shall be pen::ltted in its discretion to fix the rates for special services. (2) Wheelchair Passenger Transportation Vehicle rates: (a) Response to call, $9.00. (b) Each additional passenger (from and to same addresses) $4.50. (c) Each mile or fraction thereof - one way, $ .65. (d) Sunday and legal holiday charge. $5.00. (e) Night service charge, 7:00 p.m. to 7:00 a.m., $5.00. (f) Waiting time or standby time after first 15 minutes at request of or for the benefit of the passenger for each 15 minute period or fraction thereof, $5.00. (g) Passengers covered by ~edi-Cal will be charged and billed at the State's schedule of maximum allowances. 7. Public Safety, Animal Control - The City provides for the regulation of animal as required by the Health and Safety Codes and City Ordinances. The City presently provides animal control to this County area under a present contract. Once annexed, the cost will be absorbed in the City's tax rate. There are present plans for expanding the City's animal shelter facilities located at 333 Blood Bank Road that will allow for an additional 12 new holding pens. Currently, the City has 4 full time animal control officers, equipped with 4 appropriate vehicles. 8. Street Construction, ~Bintenance and Sweeping - Upon annexation, the City would continue the regular maintenance and sweeping progra~, which is paid for out of the City's tax rate. If any new streets are constructed within this proposal, the cost will be borne at the developer's expense and shall be constructed to City Standards. 9. Lighting - The City of San Bernardino's Electrical Division of the Public Works Department maintains the street lighting once annexed. Any expansion of the present services or future additional lighting would be contingent upon development of the area, which cost would be paid at the expense of the developer. . 10. Flood Control - This service is provided by the County and remains so after annexation to the City. 11. Parking - The public parking provided within this project area is on street parking, which is regulated under City Ordinances and enforced by the City Police Department, and the off-street parking provided within the present shopping center complex. l~en the area is further developed, the City's Planning and Engineering. Departments will make the appropriate recommendations for additional off-street parking in conformance to City Ordinances that regulate parking for the type of development which would be applicable. The cost of off-street parking would be borne by the developer and/or property owners. 12. Waste water: Collection, Treatment, Disposal and Reclamation - The East San Bernardino County ~Iater District serves this area with an existing 10" V.C.P. main in Highland Avenue and a 15" V.C.P. in Sterling Avenue. There are at present inactive connections to the properties facing Highland Avenue within this proposal. The treatment, disposal and reclamation of wastewater is processed by the City of San Bernardino's own Treatment Plant. -3- ,0 No future extension or upgrading such a request is nade, at which time the customer requesting this change. Water Service: Acquisition, Storage and Distribution - This area lies within the East San Bernardino County Water District boundaries. Presently, there are no storage facilitieS within this project, but there are water mains. A 12" main runs along Highland Avenue and into the parking lot area and a 12~3/4" main runs along Sterling Avenue with a 4" line running west approximately 250' and 530' south of centerline of Highland Avenue with a fire hydrant connected to it. Please see Exhibit "B" for specific cost of services. 14. Cable TV - Teleprompter Cable TV Co. presently provides service to this area, upon property owner's request. The basic charge is $19.95 to install, plus $7.50 @ month service charge, plus the appropriate City utility tax of 5% on such services. 15. Public Utilities: Electricity, Gas and Telephone Service - These services are currently provided to the project area. The City will allow service to continue upon annexation under the present franchise agreements between the various utility companies. The rate structure and cost to the property owners are controlled by the Public Utilities Commission and any expansion of future services ~~ll be created by the request of developers and/or property owners. 16. Solid Waste: developed and there is has been annexed for 5 San Bernardino. Collection and Disposal - The area is currently commercially private service at the present time. However, after the area years, the refuse collection will be handled by the City of 17. Transit - Upon annexation, the bus service provided for this area will remain under the control of Omnitrans. Changes in Service may be made through the Omnitrans long range transit plan or at the request of residents in the area through 'their Councilman or directly to Omnitrans. Such change must have approval of the City Council. The Short Range Transit Plan calls for continuation of the present service levels on these routes. Service level changes are made twice yearly, in January and June, and are initiated at,the request of the City Councils and/or Omnitrans planning staff. Public transportation in the Omnitrans service area is finnncec' by the following fund sources: (1) Section 5 of the Urban Mass Transportation Act. , (2) Article 4 of the Transportation Development Act.
