Resolution ratifying the submittal of four (4) project applications for FY 2003/ 2004 Hazard
Elimination Safety Program (HES) Funding
Background:
The Transportation Efficiency Act for the 215` Century (TEA-21) of 1998 requires that ten percent of
the apportioned Surface Transportation Program (STP) funds be made available for safety programs
as defined by Section 130- Railway-Highway Crossing Program and Section 152- Hazard
Elimination Program of Title 23, United States Code.
The HES Program provides funds for safety improvements on any public road, any public surface
transportation facility, any publicly owned bicycle or pedestrian pathway or trail, and for any traffic
calming measure. These funds serve to eliminate or reduce the number and severity of traffic
accidents at locations selected for improvement. It is anticipated that District 8 agencies, which
includes Riverside and San Bernardino Counties, may receive a total of $925,000 to fund their HES
Projects this fiscal cycle.
The City has nominated four projects for the FY 2003/2004 HES funding cycle. These project
locations have been considered for safety improvement upgrades as a result of council, citizen, and
SBPD requests. The application deadline was February 15, 2002 and given the quick deadline and
staff time necessary to prepare other infrastructure grant applications staff was unable to present the
request to Council in advance. The projects, which require a 10% local match, are listed as follows:

Financial Impact:
Beginning with the 2003/2004 Federal Fiscal Year (FFY) HES Program Plan, the maximum federal
reimbursement ratio for any HES project will be 90 percent and the maximum federal reimbursement
amount for any project will be $360,000. In accordance with that requirement, the City would be
obligated to allocate local matching funds of $19,737.50 if all of the projects are awarded. It is
anticipated that the local match would come from the Traffic Systems Construction Fund in the FY
02/03 budget.

Applicants seeking HES fiords must use this form. Failure to provide information that is required or to
prepare the application in accordance with general formatting instructions will result in your application
being disqualified.
This entire Application Form must be submitted, including this introductory page. Applicants should
download the Application Form from the Internet. It can be found on the Local Assistance Home Page
at www.dot.ca.gov/hq/LocalPro¢rams/. The italicized text shown on various lines throughout the
application should be removed and replaced with the appropriate information.
Limit the application to eight (8) pages plus attachments. Attachments should be grouped and numbered
to correspond with the section number in the application. Do not provide brochures and samples of
materials unless they are directly related to a response.

PROJECT LOCATION: The signalized intersection of 5`h Street at "F" Street is located
easterly of the I-215 freeway and is centered in the downtown area of San Bernardino
(see attached vicinity map,).

response to questions or statements in the application, they must be attached to the application.
I. IDENTIFICATION AND DEMONSTRATION OF NEEDS
This section requires the applicant to demonstrate the need for the project. Using the following questions and
statements as a guide, provide a detailed narrative description of the problem:
Provide some background information about the problem. How was the problem identified? How long has the
problem existed? Describe the primary cause(s) of the accidents that have occurred at the location. Given that other
problems may exist within the applicant's jurisdiction, explain why this problem was chosen for improvement. Use
whatever accident data, traffic data, community surveys, reports, plans, and other environmental conditions that may
apply.
If available, provide photographs to illustrate the problem or hazard. Include these photographs as attachments.
Maximum of six (6) photos and no video tapes, please.
2. POTENTIAL FOR PROPOSED IMPROVEMENT TO CORRECT OR IMPROVE THE PROBLEM
This section requires the applicant to describe how the proposed solution will improve the safety of the public. The
improvement may improve the safety of the motoring public, bicyclists, pedestrians, atty person with an impairment,
etc., and any combination thereof. The applicant must clearly demonstrate the connection between the problem and
the proposed solution. Using the following questions and statements as a guide, provide a detailed narcative
description of the proposed improvement:

Fifth Street at "F" Street: HES TRAFFIC SIGNAL MAST ARM PROJECT
I) Identification of problem:
The City's Traffic Engineering Section received a letter request last year from the City Police
Department to evaluate the referenced intersection. The Police Department was concerned over
the numerous and frequent calls that they received relative to accidents at this location.
There is a senior citizens center located at the northwest comer, a catholic church on the
northeast corner, a dental clinic on the southeast corner, and a motel on the southwest comer of
this intersection. Motorists travel through this intersection from the north on their way to the
Carousel Mall entrance on 4`h Street and the parking facility for the Cinema Star Theatre
complex at 4"' Street and "E" Street. This is a very busy intersection with heavy vehicular and
pedestrian traffic.
A three year accident analysis investigation was conducted by staff for this intersection. The
total daily volume of vehicles entering this intersection is approximately 25,000. There have
been fifty-four (54) reported vehicular accidents between January 1, 1998 and December 31,
2000 in or adjacent to this intersection which is cause for concern. Thirty-four of these accidents
resulted in injuries to the driver's and passengers of the vehicles involved. Forty-four (44) of the
reported accidents were of the "right-angle" type where northbound and southbound motorists
failed to yield the right-of--way to traffic on Fifth Street.
Field analysis indicates that the traffic signal at this location does not have mast arms on "F"
Street, which is in the northbound/southbound direction at this intersection. Staff is therefore
recommending that the HES Program consider this request for mast arms as a viable solution to
reducing the right angle accidents at this location
2) Description of solution: Safety Index Project
Staff is recommending that this signal mast arm project be given consideration for HES funding
utilizing the attached Safety Index criteria. The City is also proposing to investigate the
possibility of striping left turn pockets on "F" Street to alleviate the problem with right angle
accidents an left turning conflicts at this location
3. POTENTIAL FOR TIMELY IMPLEMENTATION
This section requires the applicant to provide information on project implementation schedules.
Applicants must estimate dates for the following milestones. The estimated date to "Advertise Project" must be
within the FFY for which this project is being submitted for consideration. Note: Any work performed prior to
receiving written authorization to proceed from Caltrans will not be eligible for reimbursement.

1. IDENTIFICATION AND DEMONSTRATION OF NEEDS
This section requires the applicant to demonstrate the need for the project. Using the following questions and

2. POTENTIAL FOR PROPOSED IMPROVEMENT TO CORRECT OR IMPROV E THE PROBLEM
This section requires the applicant to describe how the proposed solution will improve the safety of the public. The
improvement may improve the safety of the motoring public, bicyclists, pedestrians, any person with an impairment,
etc., and any combination thereof. The applicant must clearly demonstrate the connection between the problem and
the proposed solution. Using the following questions and statements as a guide, provide a detailed narrative
description of the proposed improvement:
Describe how the proposed project corrects or improves the naffic safety at or near the project site? Justify your
response.
Discuss how the proposed improvement is the best, most cost effective solution to the problem.
Describe options or alternatives that were considered.
30th Street at "E" Street: HES TRAFFIC SIGNAL UPGRADE PROJECT
1) Identification of problem:.
The City's Traffic Engineering Section has received numerous letter requests over the past
few years for us to evaluate the referenced intersection for protected left-turn signals on "E"
Street. Most of the requests are for northbound traffic that desire to access the westbound Rte
259 connector onramp that is located on 30`" Street, westerly of "E" Street. The Police
Department has also requested that we evaluate this intersection due to the frequent calls that
they receive relative to accidents at this location.
The intersection is bounded by a convenience mart on the northwest corner, a Union 76 gas
station on the northeast corner, and the Rte. 30 cross town freeway on the south side of the
intersection. Motorists travel through this intersection from the north end of town on their way to
the Central Business District and businesses on Highland Avenue. They also travel northerly on
"E" Street with destinations such as the California University campus and the Verdemont
residential community. This is a very busy intersection with heavy vehicular and pedestrian
traffic as "E" Street is a primary north/south arterial that carries over 20,000 vehicles on a daily
basis. Thirtieth Street carries approximately 15,000 vehicles on a daily basis.
A three year accident analysis investigation was conducted by staff for this intersection. There
have been thirty-eight (38) reported vehicular accidents between January 1, 1998 and December
31, 2000 in or adjacent to this intersection which is cause for concern. Twenty of these accidents
resulted in injuries to the driver's and passengers of the vehicles involved. Twelve (12) of the
reported accidents were of the "failure to yield the right of way" type where northbound and
southbound motorists failed to yield the right-of--way to traffic on "E" Street.
Field analysis indicates that the traffic signal at this location might warrant
protected/permissive left turn signals on "E" Street, which is in the northbound/southbound
direction at this intersection. Staff is therefore recommending that the HES Program consider this ',
request for protected/permissive lets turn signals as a viable solution to reducing the left turning
accidents at this location.
2) Description of solution: Safety Index Project
Staff is recommending that this signal upgrade project be given consideration for HES funding
utilizing the attached Safety Index criteria.

Re: 30th Street at "E" Street Left-Turn Signal Arrow Request
Dear Mrs. McKown:
Per your letter request dated April 1~5, 1993, we have analyzed
the referenced intersection to determine if left-turn signal
arrows on "E" Street are needed. At this time, we find that
the guidelines established for installation of left-turn
signals (i.e., high intersection accident rate, ratio of
left-turning vehicles to opposing through traffic) have not
been met.
However, there is significant short term evening peak hour
left-turn congestion on "E" Street. We may be able to
alleviate this congestion by increasing the signal timing on
"E" Street at this location to allow for more effective left
turn traffic flow.

1. IDENTIFICATION AND DEMONSTRATION OF NEEDS

apply.

2. POTENTIAL FOR PROPOSED IMPROVEMENT TO CORRECT OR IMPROVE THE PROBLEM
This section requires the applicant to describe how the proposed solution will improve the safety of the public. The
improvement may improve the safety of the motoring public, bicyclists, pedestrians, any person with an impairment,
etc., and any combination thereof. The applicant must clearly demonstrate the connection between the problem and
the proposed solution. Using the following questions and statements as a guide, provide a detailed narrative
description of the proposed improvement:

Discuss how the proposed improvement is the best, most cost effective solution to the problem.

Pepper Aveuue at Rialto Avenue: HES TRAFFIC SIGNAL UPGRADE PROJECT
1) Identification of problem:
The City's Traffic Engineering Section was requested by 3rd Wazd Councilman, Mr. Gordon
McGinnis, to evaluate the referenced intersection for protected left-turn signals on Pepper
Avenue (see attached letter). The Police Department has also requested that we evaluate this
intersection due to the frequent calls that they receive relative to accidents at this location.
The intersection is bounded by gas station/ convenience marts on the northeast and southeast
comers, residential development on the northwest corner, and an undeveloped parcel on the
southwest comer of the intersection. Motorists (and high school students) travel through this
intersection from the north end of town on their way to the I-10 Freeway, Arrowhead
Community Hospital, and Rialto High School which is located at the southwest corner of Mill
Street at Pepper Avenue. They also travel northerly on Pepper Avenue with connections to
Foothill Boulevazd (Route 66) and Base Line Road. This is a very busy intersection with heavy
truck traffic (6%) and elementary/high school pedestrian traffic as Pepper Avenue is a primary
north/south arterial and designated truck route that carries over 20,000 vehicles on a daily basis.
Rialto Avenue is also designated as a major arterial that carries approximately 15,000 vehicles
on a daily basis. This roadway is a major surface street connector between the cities of Rialto and
San Bernardino.
A three yeaz accident analysis investigation was conducted by staff for this intersection. There
have been thirty-two (32) reported vehicular accidents between January 1, 1998 and December
31, 2000 in or adjacent to this intersection which is cause for concern. Fourteen of these
accidents resulted in injuries to the driver's and passengers of the vehicles involved. Seventeen
(17) of the reported accidents were of the "failure to yield the right of way" type where left-
tuming motorists failed to yield the right-of--way to through traffic.
Field analysis and peak hour turning movement counts indicate that the traffic signal at [his
location might warrant protected/permissive left turn signals on PepperAvenue, as well as Rialto
Avenue, at this intersection. Staff is therefore recommending that the HES Program consider this
request for protected/petmissive left turn signals as a viable solution to reducing the left turning
accidents at this location.
2) Description of solution: Safety Index Project
Staff is recommending that this signal upgrade project be given consideration for HES funding
utilizing the attached Safety Index criteria.

As a follow up to your request to evaluate the subject intersection, our office hoc
conducted an engineering study to determine if left-tum arrows aze justified.
The study concludes that left-tum arrows are warranted on both streets. However,
adding the arrows can not be achieved without a major upgrade to the entire signal,
since the existing signal hardware is very old and substandard. The preliminary cost
estimate for modifying this signal is $66,000.00.
Since no funding source has been identified for this project, we will recommend this
project be include it in the City's Capital Improvements Program (C.I.P.) for possible
funding.

Hills Middle School, located on the east side of Little Mountain Drive and south of
Morgan Road (see attached vicinityr man)

SHANDIN HILL MIDDLE SCHOOL: HES PEDESTRIAN SIGNAL PROJECT
1) Identification of problem:
The City's Traffic Engineering Section received a school pedestrian safety letter request last
year from a parent whose child attends Shandin Hills Middle School. The school administrators
are also in concurrence with this request. The parent had a concern regarding pedestrian safety in
front of the school and requested that staff evaluate the pedestrian crosswalk on Little Mountain
Drive at Morgan Road. The school is located on the east side of Little Mountain Drive, southerly
of Kendall Drive. The school boundaries are quite large as the school children are expected to
travel up to 1.5 -2.0 miles (one-way to school. The school zone boundaries are generally shown
as being University Parkway (west); 48`" Street (north); 27'h Street (south); and Kendall Drive to
the east. Shandin Hills School is a continuous yeaz-round (6-8) middle school. The school was
constructed to accommodate 100 students. There are presently about 1300 students.
The parent's request was subsequently handled by traffic engineering staff. Little Mountain
Drive is a major arterial that has a relatively steep downgrade as motorists approach the school
from the south. This street carries approximately 8,000 vehicles in a twenty-four (24) hour period
in this area. There is a posted speed limit of 45mph on Little Mountain Drive adjacent to the 25
mph school zone. All of the appropriate school signs and pavement markings are in place. There
is a previously mentioned school crosswalk on Little Mountain Drive, just south of where
Morgan Road would intersect Little Mountain Drive (Morgan Road is closed at Little Mountain
Drive shown on the attached vicinity map).
There have been five (5) recorded accidents between January I, 1998 and December 31, 2000
(please see 3 year accident history attachment). The accidents are scattered throughout the
general area of the school. However, this includes a pedestrian related accident that occurred at
the referenced school crosswalk on Little Mountain Drive, south of Morgan Road
Staff conducted a "student crosswalk count" at the subject location. The count was taken at
the 2:45pm afternoon dismissal and a total of 150 students were counted. This crosswalk is along
a primary route for school children who live in residences westerly of the school between
Kendall Drive and University Pazkway.
Description of solution: Work Type Improvement Project
In view of the large number of students that cross Little Mountain Drive at this location every
school day, staff evaluated the crosswalk utilizing Caltrans' School Area Traffic Sienal Wazrant
criteria. This location meets the school protection warrant for a pedestrian activated traffic signal
(see attachment). The school district does not presently authorize the use of paid crossing guards
at middle schools. Staff is therefore recommending that this pedestrian signal project be included
in the City's five year Capital Improvements Program to alleviate the safety concerns of the
parents and school officials at Shandin Hills Middle School.
