One to the contract with Grand Pacific Contractors, Inc. for Installation of Reinforced Concrete
Box Culvert at 2128 Orange Street (SD04-l6), per Plan No. 11568
Background:
On August 7, 2005, the Mayor and Common Council allocated funds, as part of the FY 2005/06
Capital Improvement Program (CIP), for Orange Street failed rock-lined channel reconstruction
(SD04-l6).
On June 5, 2006, the Mayor and Common Council adopted resolution awarding contract in the
amount of $45,300 to Grand Pacific Contractors, Inc. for Installation of Reinforced Concrete
Box Culvert at 2128 Orange Street (SD04-l6), per Plan No. 11568.
This project replaced a portion of a rock-lined ditch with a reinforced concrete box culvert with
new side walls to transition back to the existing channel walls. The rock-lined channel was
designed to be repaired at one location due to damage from tree roots.

During construction, it was necessary for the contractor to remove an additional tree and three
stumps in order to continue work. A wrought iron fence had to be added along each side ofthe
channel crossing for safety reasons.
The existing rock-lined channel was found by the contractor to be filled with mud and silt, which
made assessment of the damage to the channel impossible. After removal of the mud and silt, the
contractor found additional channel damage that needed to be repaired. As a result, 45 linear feet
of the drainage holes were added to facilitate drainage of the adjacent properties. A credit for bid
Item No.5 "Replace Channel Wall" was taken because the item was made unnecessary by the
decision to reconstruct the entire channel along the property frontage.
Because of the mud and silt buildup in the channel it was impossible to accurately assess damage
to the channel during the design phase of the project. After removal of the mud and silt by the
construction contractor, it became apparent that extensive additional work was necessary in order
to have a safe, functional channel.
Change Order No. One, in the amount of $33,424.00, consists of the following changes:

Item No.1-I: Increased clearing and grubbing.
Item No. 1-2: Installation of 106 linear feet of wrought iron fence, which was not in the
original contract.
Item No. 1-3: Removal and replacement of 45 linear feet of channel; which was too badly
deteriorated to join to the new channel.
Item No. 1-4: Core-drill drain holes for improved drainage from adjacent property.
Item No. 1-5: Credit for Bid Item No.5 the need for which was negated by Change Order
Item No. 1-3.

Financial Impact:
The FY 2006-07 CIP budget included $89,000 in Acct. No. 248-368-5504-7573 for this project,
which will be adequate funds to cover the cost of this change order and all other costs for this
project. At this time, it is anticipated that all $89,000 of budgeted funds will be used for this
project.

Core Drill Drain Holes
Item No. 1-4 The top of the newly constructed channel blocked on-site drainage, which created a
water ponding situation. Therefore, the contractor was directed to core (4) 3-inch holes
in the channel wall for drainage.
Credit for Bid Item No 5
Item No. 1-5 Bid Item No. 5 was to replace a portion of the existing channel wall. With the
reconstruction of the entire channel, this item was no lon~er necessarv.

This change order represents 73.78% of the original contract amount.
Section 3-2.1 of the Standard Specifications states that an increase in the total contract amount of more
than 25% must be accomplished by written Supplemental Agreement between the Contractor and
Agency, unless both parties agree to proceed with the change by Change Order.
The signatures hereon indicate concurrence that accomplishing this change by change order is acceptable
to both parties.
