Synopsis of Previous Council Action:
01110/05 Mayor and Common Council adopted Resolution No. 2005-7 awarding contract in the amount
of$2,181,877.00 to Matich Corporation for construction of Campus Parkway (SS04-01) and
installation of a traffic signal at Kendall. Drive and Campus Parkway (TC04-07) per Plan Nos.
10952 & 10232.
11/21/05 Mayor and Common Council authorized the Director of Development Services to execute
Change Order No. One to contract in the amount of $82,032.00 with Matich Corporation for
construction of Campus Parkway (SS04-0 I) and installation of a traffic signal at Kendall
Drive and Campus Parkway (TC04-07), per Plan Nos. 10952 & 10232 and authorized the
Director of Finance to amend the FY 2005/06 budget.
12/05/05 Mayor and Common Council authorized the Director of Development Services to execute
Change Order No. Two in the amount of $330,000.00 to contract with Matich Corporation for
construction of Campus Parkway (SS04-0 I) and installation of a traffic signal at Kendall
Drive and Campus Parkway (TC04-07), per Plan Nos. 10952 & 10232 in the amount of

Subject:
Authorization for the Director of Development Services or designee to execute Change Order
No. Four to Contract with Matich Corporation for construction of Campus Parkway (SS04-01)
and installation of a traffic signal at Kendall Drive and Campus Parkway (TC04-07), per Plan
Nos. 10952 & 10232.
Background:
On January 10, 2005, the Mayor and Common Council awarded a contract in the amount of
$2,181,877.00 to Matich Corporation for construction of Campus Parkway (SS04-01) and
installation of a traffic signal at Kendall Drive and Campus Parkway (TC04-07) per Plan Nos.
10952 & 10232.
On November 21, 2005, the Mayor and Common Council approved Change Order No. One in
the amount of $82,032.00 which authorized the following changes: installation of an additional
catch basin and connector pipe on Campus Parkway near Kendall Drive; modification of
foundations for streetlights; construction of parallel parking on Campus Parkway; and relocation
of a 6" waterline along the north side of Kendall Drive.
On December 5, 2005, the Mayor and Common Council approved Change Order No. Two in the
amount of $347,900.94 which authorized the following changes: installation of a parkway drain;
construction of a sliver fill and embankment for Northpark Boulevard near Campus Parkway;
modification of grading on Northpark Boulevard; removal of sandbags along Northpark
Boulevard; re-grading the existing subgrade on Campus Parkway for placement at curb and
gutter; placement of fill in planter areas; equipment standby time; re-grading of east end of
Campus Parkway; and additional surveying.
On April 30, 2006, the Mayor and Common Council approved Change Order No. Three in the
amount of $330,000 to compensate the contractor for substantial increases in costs of materials
and labor that occurred due to a delay issuing a "Notice to Proceed."
Change Order No. Four authorizes compensation to the contractor for various items of work,
which were not included in the original contract but were essential to insure a safe, functional
roadway. Please see the attached change order for details of the changes.
Construction of the project is complete. Approval of this change order will allow the project to
be closed out.
Financial Impact:
Adequate funding for Change Order No. Four is available in Account No. 129-367-5504-7007

JUSTIFICATION:
Proiect Delav
During construction, the grades on Campus Parkway did not match the grades at the
4-1 intersection with Northpark Boulevard. The contractor's equipment was required to
standby pending redesign of the profiles and re-grading of Campus Parkway and
Northpark Boulevard.
Additional Irril!ation at Northpark Bonlevard. south of Ash Street
4-2 Modifications to the median in Northpark Blvd. required installation of new valves,
wiring and irrigation mainline relocation. This work was not included in the original bid
documents.
Storm Drain at Northpark Boulevard and Ash Street.
46 linear feet of 24-inch storm drain pipe was installed at the intersection of Northpark
4-3 Boulevard and Ash Street to facilitate draining Ash Street. This pipe was not included in
the original bid documents and the need for it was not discovered until after the contract
was awarded.
Additional Catch Basin at Northpark Boulevard
A catch basin was added on Northpark Boulevard at Ash Street to control sheet flow
4-4 across Ash Street at Northpark Boulevard. This catch basin outlets into the drain pipe
identified in Item 4-3. This additional catch basin and connector pipe was not included in
the original bid documents. The need for the drain was discovered after the contract was
awarded.
Adiust Traffic Sienal Pole.
4-5 The traffic signal pole specified for the northwesterly comer of Campus Parkway and
Kendall Drive was too tall and conflicted with overhead power lines along Kendall Drive.
It was necessary for the contractor to shorten the pole.

Move Traffic Silmal Pedestal.
Subsequent to receipt of bids for the project, the adjacent tract developer agreed to
dedicate land and develop a park (Chancellor's Park) at the northeasterly comer of
Campus Parkway and Kendall Drive, A service pedestal for the traffic signal was
4-6 installed on the northerly side of Kendall Drive easterly of Campus Parkway,
Unfortunately, the pedestal, which stands approximately 3 feet above ground, created a
visual obstruction not consistent with the park's design theme. James Funk, the Director
of Development Services at the time, directed that the pedestal be removed and relocated
clear of the park frontage.
Raise Fire Hvdrant on Fairview Drive.
After start of construction it was discovered that Fairview Drive, a private street that
4-7 serves a maintenance building and parking lot on the campus of California State
University San Bernardino, was designed too steep for access by maintenance trucks. It
was necessary to substantially raise the grade, which required that an existing fire hydrant
on Fairview Drive be raised to match the new grade.

After start of construction it was discovered that Fairview Drive, a private street that
4-8 serves a maintenance building and parking lot on the campus of California State
University San Bernardino, was designed too steep for access by maintenance trucks. It
was necessary to substantially raise the grade, which required that foundations for the
existing streetlights along Fairview Drive be raised to match the new grade.
Modifv Backflow Preventer Assemblv.
4-9 The two backflow preventer assemblies at Ash Street and Valles Street did not meet
Water Dept. requirements. They were modified and reassembled on site to comply with
Water Deot. soecs.
Additional Sil!ninl! and Strioinl!.
4-10 After start of construction it was discovered that an additional 93 "No Parking" signs and
additional striping needed to be added in the interest of traffic safety. The additional
signing and striping was not included in the original bid documents.
Additional Bid Item No. 40 "Irril!ation Pipe Sleeves"
After start of construction, it was discovered that the quantity for Bid Item No. 40
4-11 "Irrigation Pipe Sleeves" had been underestimated. The bid documents indicated that
1,580 linear feet of irrigation pipe sleeves would be needed, however, a total of 3,115
linear feet of oioe sleeves was reauired.
Additional Bid Item No. 33 "Remove Curb & Gutter at Northpark Boulevard and
Fairview Drive"
4-12 After start of construction, it was discovered that the quantity for Bid Item No. 33
"Remove Curb & Gutter at Northpark Boulevard and Fairview Drive" had been
underestimated. The bid documents indicated that 50 linear feet of curb & gutter would
be removed, however, a total of 833 linear feet of curb & \!Utter had to be removed.
Replace Fairview Drive Landscapinl! and Irril!ation.
After start of construction it was discovered that Fairview Drive, a private street that
serves a maintenance building and parking lot on the campus of California State
4-13 University San Bernardino. was designed too steep for access by maintenance trucks. It
was necessary to substantially raise the grade. Raising the grade resulted in loss of all
existing landscaping and irrigation along the parkways and slopes of Fairview Drive. It
was necessary for the contractor to replace all the landscaping and irrigation along
Fairview Drive.

Raise Manholes alonl! CamDus Parkwav
Sewer and storm drain improvements were installed in Campus Parkway by the developer
of the adjacent tract. The elevation of the tops of the manholes was set based on the rough
4-14 grade, which was also done by the developer. The requirement to adjust the developer-
installed manholes to match the finished street surface was not included in the original bid
documents for Campus Parkway. It was necessary for the contractor to adj ust six (6)
manholes to grade.
Chanl!e Overhead Sil!n on Traffic Sil!nal at CamDus Parkwav and Kendall Drive.
After start of construction, it was discovered that a revision to a sign, which is suspended
4-15 from the signal mast arm, was needed to prohibit left turns and V-turns for northwesterly
bound traffic at the intersection of Kendall Drive and Campus Parkway. This requirement
was not included in the original bid documents for the signal.
Credit for work authorized but not needed in Dreviouslv Daid C.C.O. No. Two.
Change Order No. 2 authorized payment based on an estimated cost of approximately
4-16 $304,000 for re-grading the intersection of Northpark Boulevard and Campus Parkway
and adjacent areas to correct a mismatch in grades. The estimated cost was in excess of
the actual cost of the work performed based on time and materials reports; therefore, the
contractor has provided a credit to the contract.
Install LandscaDe Drains in CamDus Parkwav & NorthDark Boulevard Median
Islands.
4-17 The median islands on Campus Parkway and Northpark Boulevard were designed without
drainage capabilities. The subgrade is impermeable and water does not percolate well in
this area; therefore, it was necessary to install drains through the curbs to properly drain
the median islands.
Increase Wire Size for Streetlil!hts alonl! CamDus Parkwav and NorthDark
Boulevard.
4-18 The wire size indicated on the design plans was inadequate due to the large number of
lights and length of conduit runs. The wire size was increased to avoid excessive voltage
drop, which could result in poor lighting of the roadways.
Relocate Existinl! 6-inch Water Main in CamDus Parkwav near Kendall Drive.
During construction, a 6-inch water main was discovered to be too shallow and in conflict
4-19 with the proposed structural section of Campus Parkway. San Bernardino Municipal
Water Department insisted that the water line be lowered 14 inches to clear the structural
section of Campus Parkway.
ReDlace Interconnect Cable between Wal!onwheel Road and Campus Parkway.
After start of construction, it was discovered that the existing hardwire interconnect cable
4-20 between Wagonwheel Road and Campus Parkway was too short. Therefore, it was
necessary to replace the entire interconnect cable with a longer cable in order to
interconnect and coordinate the two signals.
Install Two (2) Water Valve Covers.
The original bid documents called for the new water valve covers to be round. This
4-21 design was correct at the time the plans were prepared. However, the San Bernardino
Municipal Water Department advised that they now require triangular covers. As a result,
it was necessary for the contractor to break out two round covers, which had been
installed, and replaced them with triangular covers.
Increase Bid Item No.3 "Plant I-Gallon Shrub"
4-22 The number of I-gallon shrubs had to be increased to provide coverage specified in the
bid documents. Bid Item No.3 of the additive bid were increased by a total quantity of
1,443 plants, which increased the total cost of the two items by $15,873.00.
Credit for Chanl!es to Entry Monument Sil!n.
4-23 During final stages of construction of the entry monument sign located on Campus
Parkway near Kendall Drive, trim was determined to be not needed and was deleted. This
resulted in a credit from the contractor.
