RELATING TO THE NUTRITION FOR SENIORS PROGRAM. 3 4 BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL OF THE CITY OF ,SAN BERNARDINO AS FOLLOWS: 5 6 SECTION 1. The Mayor of the City of San Bernardino is hereby 7 authorized and directed to execute on behalf of said City a First 8 Amendment to Agreement with San Hidalgo Institute, Inc., relating 9 to the nutrition for seniors program, a copy of which is attached 10 hereto, marked Exhibit "A" and incorporated herein by reference as 11 fully as though set forth at length. 12 I HEREBY CERTIFY that the foregoing resolution was duly 13 adopted by the Mayor and Common Council of the City of San J4 Bernardino at a meeting thereof, held regular 15 on the 5th day of , 1983, by the following July 16 vote, to wit: 17 AYES: Council Members Castaneda, Eeilly. HeTnanJe~; 18 .;"~,arks v Quiel, Yrazier ~ S'tri,cJ:::ler 19 NAYS: None 20 ABSENT: None 21 22 ~hl!J/~ / City Clerk 23 The foregoing resolution is hereby approved this ,/'/(' day 24 of Jul.y , 1983. 25 26 f San Bernardino Approved as to form: 27 C~~ 28 1 FIRST AMENDMENT TO AGREEMENT 2 (Nutrition for Seniors Program) 3 THIS FIRST AMENDMENT TO AGREEMENT is entered into between the 4 CITY OF SAN BERNARDINO, a municipal corporation, referred to as 5 "City", and the SAN HIDALGO INSTITUTE, INC., referred to as 6 "Institute". 7 City and Institute agree as follows: 8 1. Recitals. 9 (a) City and Institute entered into an Agreement on ]0 July 8, 1983 ("the Agreement"), relating to managing a senior 11 citizen nutrition site at the Meadowbrook Housing Complex for 12 fiscal year 1982-1983. 13 (b) The parties now desire to amend the Agreement to 14 provide for a one-year extension of its term and to set forth the 15 number of meals to be provided, and the budget for the total 16 amount reimburseable to Institute for actual expenses incurred for 17 the fiscal year 1983-1984. 18 2. Amendments. 19 (a) Subparagraph (a) of Paragraph 1 of the Agreement is 20 amended as follows: 21 "(a) Manage, conduct and otherwise perform all activities 22 and supporting social services for the Nutrition for Seniors 23 Program as approved by the Department of Aging grant to the City, 24 for a period beginning July 1, 1983, and ending June 30, 1984. 25 planned Meals. A meal, in conformity with a menu approved by 26 City, will be prepared and served five days per week for a minimum 27 of 252 days per year at the Meadowbrook Nutrition Site." 28 (b) Subparagraph (e) of Paragraph 1 of the Agreement is 1 I, Ii II i: i ~amended as follows: Ii "(e) Prepare up to 75 congregate meals per day and prepare I: iiup to an additional 45 meals for home delivery on each Monday." Ii II (c) Subparagraph (a) of Paragraph 2 of the Agreement is , iiamended as follows: " 2 3 4 5 6 ~ "Ia) Pay to the Institute the actual expenses incurred by I 7 ~Institute for those items listed on Attachment B, annexed hereto, i: 8 i1not to exceed the total sum of $34,395 for the term of this 9 ~agreement, payable in one initial installment of one-twelfth !! 10 Ii I $2,866), and eleven subsequent increments based upon invoices and :! i; :supporting documents in accord with Attachment B - line item , I ibudget. Institute invoices for salary and fringe benefits will IShOW position, employee name, hourly rate and hours worked in Iprovision of meals per pay period. The initial payment will be Ipaid upon the execution of this agreement. Other Provisions.
