RESOLUTION OF THE CITY COUNCIL OF SAN BERNARDINO AWARDING A
PURCHASE ORDER TO CREST CHEVROLET FOR FURNISHING AND DELIVERY OF,
FOUR (4) EACH 1997 CHEVROLET 1/2 TON MINI PICK-UP TRUCKS TO BE
UTILIZED BY THE PUBLIC WORKS DEPARTMENT OF THE CITY OF SAN
BERNARDINO IN ACCORDANCE WITH BID SPECIFICATION F-97-09-C.
BE IT RESOLVED BY THE MAYOR AND COMMON COUNCIL OF THE CITY
OF SAN BERNARDINO AS FOLLOWS:
SECTION 1.
That Crest Chevrolet is the lowest and best
responsible bidder for the furnishing and delivery of four (4)
each 1997 Chevrolet 1/2 Ton Mini PiCk-Up Trucks in accordance
with Bid Specification F-97-09-C for the total amount of
$57,808.01; pursuant to this determination, the Finance
Department is hereby authorized and directed to issue a purchase
order for said four (4) 1/2 Ton Mini Pick-Up Trucks for said
lowest and best responsible bidder; such award shall only be
effective upon issuance of a Purchase Order by the Finance
Department; and all other quotations therefor are hereby
rejected.
SECTION 2.
The authorization to execute the above
referenced purchase order is rescinded if it is not issued
within sixty (60) days of the passage of this resolution.
I HEREBY CERTIFY that the foregoing resolution was duly
adopted by the Mayor and Common Council of the city of San
Bernardino at a joint regular

GENERAL
These specifications are intended to describe
the city of San Bernardino's need for..aulmi~

operation. All specifications are minimum. Any
quotations under this shall include all standard
equipment listed in published sales brochures as
well as options required to meet this
specification. Bidders are expected to meet or
exceed these specifications in their entirety.
Separate bids are requested for the two (2)
following options:

WARRANTY
Shall have a minimum of three (3) year bumper-

include in their bid extended warranty to cover
a full five (5) years, and include information
brochures to cover.
REGISTRATION
The successful bidder is to obtain and include
in the delivery price, all registration
documents and licenses. The City will provide
the necessary application for exempt plates.
DELIVERY
Delivery is to be F.O.B. to the City of San
Bernardino City Yard at 182 S. Sierra Way, San
Bernardino, CA Attention: Fleet Manager (909)
384-5220. Three (3) days notice, prior to
delivery, is to be given.

BIDDER PRE-QUALIFICA TIONS
INTRA-LA TA AND LONG DISTANCE CALLING
Although competitively priced telecommunication services is
important to the City, the City also expects a high level of
cooperation with its telecommunication provider(s) in all areas
of interaction with the City. One significant area of interaction
is the collection of the City'S utility users tax on
telecommunication services provided by the
telecommunications provider. As a pre-qualification for this bid,
each bidder is, therefore, asked to provide an executed
statement that states:
This will confirm that I have received a copy of Chapter 3.44
of the San Bernardino Municipal Code and the City'S UUT
Administrative Rules in the bid package. As a
telecommunication service provider, my firm agrees to
abide by the duties and responsibilities of such providers
in collecting and remitting taxes as set forth in Chapter
3.44. In addition, my firm agrees to abide by the City'S UUT
Administration Rules. In particular, my firm agrees to
comply with subsections G and H of Part I of those Rules
within thirty days of the date of this statement, and,
within sixty days, will permit the City'S representative to
review and correct the geocoding (billing address) of
customers within and adjacent to the City of San
Bernardino.

Technical Specifications
Bid Specifications :INTRA-LATA AND LONG DISTANCE CALLING
1. The purpose of this RFP is to award a contract between the City and a
selected vendor for the purpose of processing calls from a variety of
telephone systems and locations, within the City, to areas within the LATA
(Local Access & Transport Area 5) where the call was originated, and long
distance (areas outside LATA 5). Local calls and Zone 1, 2 and 3 calls will not
be considered for this proposal.
Bidder's responses to this RFP should be based on the establishment of a
service contract for a period of three (3) years with an option by the City,
at the end of that three (3) years, to extend the contract for one (1)
additional three (3) year period.
2. The primary service for City locations are NEC PBX'S. Service for small
locations is provided by a variety of telephOne systems such as CENTRANET,
Key systems and 1MB lines. With the exception of 1MB lines, all systems are
equipped with "Automatic Route Selection" for the placing of calls on
selected routes.
Primary access for Intra.LATA and Long Distance services is provided to the
City Hall PBX via T-1 service from the local Central Office. 99% of all Intra-
LATA and Long Distance traffic for the entire City is routed through the City
Hall PBX.
Total estimated Intra-LATA and Long Distance call volume, based upon a six
month average (January 1996 to June 1996), is 30,000 minutes per month.
3. If, during the life of the contract, the successful bidder'S rates decrease,
the City will always receive the most favorable pricing. Rates will not
increase for the City at any time during the life Of the contract.
4. The bidder shall answer all questions listed in section 6 of this RFP.
Failure to answer each of the questions will disqualify the bidder. If the
answer to any question is YES, the bidder shall answer and move onto the
next question. If the answer to any question is YES, yet the bidder feels
additional information is required, the question should be answered and
denoted with an asterisk. The additional information must be provided on

a separate sheet of paper. If any answer Is NO, an explanation must be
provided on a separate sheet of paper. separate sheets of paper must be
clearly labeled with the Question number. Any Question requiring additional
information, or not labeled clearly, can disqualify the bidder.
5. Proposals submitted In response to this RFP will be evaluated by a review
committee. The evaluation of proposals will be based totally upon the
information provided within the Response Questionnaire, section 6 of this
RFP.
Selection of the successful bidder will be determined by the following
criteria:
A. Overall cost to the City
B. Overall responsiveness to the requirements of the RFP; as
determined through the evaluation process
C. References from current customer subscribers
