Such written determination shall include a description of the consultant's duties and, based upon that description, a statement of the extent of disclosure requirements. Disclosure Category 2: (Schedule A-2) Investments, Income/Loans, Real Property and Assets of Business Entities/Trusts (ownership interest is 10 percent or greater). Disclosure Category 3: (Schedule B) Interests in real property located within the jurisdiction of the City of San Bernardino in which you, your spouse, or your dependent children had a direct, indirect, or beneficial interest aggregating $1000 or more during the reporting period; rental income and loans. Disclosure Category 4: (Schedule C) Gross income and your community property interest in your spouse's gross income aggregating $250 or more received from any source located in or doing business within the jurisdiction of the City of San Bernardino; and, job title or business position you held with a business entity (director, officer, partner, trustee, employee, or any management position) during the reporting period. Disclosure Category 5: (Schedule D) Loans received or outstanding during the reporting period which aggregate $250 or more from a single source located in or doing business within the jurisdiction of the City of San Bernardino.
